前房角狭窄患者白内障术后前房角变化

目的观察前房角狭窄患者行白内障超声乳化术后前房角及其眼压的变化。方法对32例36眼前房角狭窄的白内障患者行超声乳化吸除＋后房型人工晶体植入术，测量并比较术前及术后前房角的宽度、前房角入口的角度和眼压。结果术前房角：4／4周N,2眼，4／4周N,18眼，4／4周Nw16眼，术后房角：4／4周宽房角（W）20眼，大于1／2周宽房角（W）9眼，窄房角（N1～N1）7眼，与术前相比有统计学差异（P〈0．01）。房角入口：术前入口〈10°者30眼，10°～20。者6眼，术后36眼入口均〉20°，与术前相比有统计学差异（P〈0．01）。眼压：术前平均眼压（18±6．41）mmHg，术后平均眼压（10．0±3．52）mmHg，与术前相比有统计学差异（P〈0．05）。结论具有前房角狭窄的白内障患者，行白内障超声乳化吸除术后，可以使关闭的前房角重新开放、变宽，对因房角关闭引起的青光眼有预防和治疗作用。

The change of anterior chamber angle in narrow angles cases after cataract extraction

[Objective] To observe the transformation of anterior chamber angles and intraocular pressure （IOP） in patients with narrow angles after phacoemulcification. Methods 32 patients （36 eyes） with cataract with narrow anterior chamber angles, including 16 cases/20 eyes with narrow chamber angles simply, 4 cases/4 eyes with forepart chronic angle closure glaucoma and 14 cases/14 eyes with acute angleclosure glaucoma, underwent cataract phacoemulcification and IOL implantation. Before and after the surgery, IOP and the width and entrance of anterior chamber angles were measured. Results There were 20 eyes with 4/4 wide chamber angles（W）, 9 eyes 1/2 wide chamber angles（W）, 7 eyes narrow chamber angles （N1- N1 ） after operations, which was significantly different form that before operation （P 〈0.01）. The entrance angles of 30 eyes were smaller than 10°, 6 eyes between 10°to 20°, and 36 eyes 〉20° after operation. The IOP before and after operation were 18±6.41 mmHg and 10±3.52 mmHg （P〈0.05）. Conclusion Phaeoemuleifieation can open closed anterior chamber in patients with cataract with narrow anterior chamber angles, and prevent and treat glaucoma.
