单操作孔电视胸腔镜肺叶切除术治疗肺结核的临床疗效研究

摘    要:目的:分析单操作孔电视胸腔镜手术(uniportal video-assisted thoracoscopic surgery,UVTAS)肺叶切除术治疗肺结核的临床应用价值。方法回顾性分析2013年1月—2015年6月在河北省胸科医院行全胸腔镜肺结核肺叶切除术患者90例,分为观察组 UVATS 肺叶切除术45例和对照组双操作孔电视胸腔镜肺叶切除术45例,将两组围手术期各临床指标、术后疼痛、并发症发生率等进行比较。结果两组患者围手术期各临床指标差异无统计学意义(P ﹥0.05);观察组术后疼痛评分、并发症总发生率均低于对照,差异有统计学意义( P ﹤0.05)。结论 UVATS 肺叶切除术治疗肺结核是安全、有效、可行的。

Clinical Efficacy of Uniportal Video-assisted Thoracoscopic Surgery in Treatment of Pulmonary Tuberculosis Pa-tients Undergoing Pulmonary Lobectomy
Abstract:Objective To investigate clinical application value of uniportal video-assisted thoracoscopic surgery (UVTAS)in treatment of pulmonary tuberculosis patients undergoing pulmonary lobectomy. Methods Clinical data of 90 pulmonary tuberculosis patients undergoing pulmonary lobectomy by UVTAS during January 2013 and June 2015 was retrospectively analyzed,and the patients were divided into 45 patients undergoing UVATS( observation group)and 45 patients undergoing diplopore video-assisted thoracoscopic surgery(control group). Perioperative clinical indexes,post-operative pain and incidence rate of postoperative complications were compared in two groups. Results There were no significant differences in perioperative clinical indexes in two groups(P ﹥ 0. 05). Postoperative pain score and total inci-dence rate of postoperative complications in observation group were significantly lower than those in control group( P ﹤0. 05). Conclusion Uniportal video-assisted thoracoscopic surgery is safe,effective and feasible in treatment of pulmo-nary tuberculosis patients undergoing pulmonary lobectomy.
