气压治疗对2型糖尿病下肢周围神经病变患者神经症状及缺血状态的效果观察

摘    要:目的:观察气压治疗对2型糖尿病下肢周围神经病变患者神经症状及缺血状态的影响。方法对2013年3月—2015年10月收治的2型糖尿病下肢周围神经病变113例的临床资料进行回顾性分析。按治疗方案分为A组(61例)与B组(52例)。两组均给予常规治疗,A组应用甲钴胺联合气压治疗,B组单纯应用甲钴胺。观察两组治疗前后血糖与血脂水平、神经症状、缺血状态的改善情况及临床疗效。结果两组治疗前后血糖与血脂水平比较差异无统计学意义(P>0.05)。治疗后两组神经系统症状评分(NSS)、密西根糖尿病性周围神经病评分(MDNS)与治疗前比较降低,腓总神经的感觉神经传导速度( SNCV)和运动神经传导速度( MNCV)与治疗前比较增大,且A组治疗后NSS与MDNS评分低于B组,SNCV、MNCV高于B组(P<0.05)。 A组治疗总有效率高于B组(P<0.05)。结论气压治疗利于改善肢体动脉缺血及全身血液循环,在2型糖尿病下肢周围神经病变患者中应用可促进神经症状缓解及改善缺血状态。

Effect Observation of Air Pressure Therapy on Neural Symptoms and Ischemic State in Type 2 Diabetes Mellitus Patients with Peripheral Neuropathy in Lower Limbs
Abstract:Objective To observe effects of air pressure therapy on neural symptoms and ischemic state in type 2 diabetes mellitus (T2DM) patients with peripheral neuropathy in lower limbs. Methods Clinical data of 113 T2DM patients with peripheral neuropathy in lower limbs admitted during March 2013 and October 2015 was retrospectively ana-lyzed. The patients were divided into group A (n=61) and group B (n=52) according to different treatment plans. Af-ter conventional therapy, group A was treated with Mecobalamin combined with air pressure, while group B was treated with Mecobalamin only. Blood glucose and blood lipid levels, neurological symptoms and ischemic state before and after treatment, and clinical effect and improvement conditions were observed in two groups. Results There were no signifi-cant differences in blood glucose and blood lipid levels in two groups before and after treatment ( P>0. 05 ) . Compared with those before treatment, values of neurological symptom score ( NSS) and Michigan diabetic peripheral neuropathy score (MDNS) were significantly lower, while sensory nerve conduction velocity (SNCV) and motor nerve conduction velocity (MNCV) of common peroneal nerve were increased after treatment in two groups (P>0. 05). After treatment, in group A, NSS and MDNS values were significantly lower, while SNCV and MNCV values were higher than those in group B (P<0. 05). The total effective rate in group A was higher than that in group B (P<0. 05). Conclusion Air pressure therapy is conductive to improving arterial ischemia in limbs and general blood circulation, and therefore it can re-lieve neurological symptoms and ischemic state in treatment of T2DM patients with peripheral neuropathy in lower limbs.
