病理诊断与内镜诊断在胃黏膜肠上皮化生诊断中的相关性分析

目的：分析病理诊断与内镜诊断在胃黏膜肠上皮化生诊断中的相关性。方法选择哈尔滨医科大学附属第一医院2012年10月—2014年10月通过病理诊断为胃黏膜肠上皮化生62例作为研究对象,回顾性分析其临床病历资料,使用内镜对胃黏膜肠上皮化生表现特点进行观察,然后比较病理检查方法与内镜检查方法的诊断结果。结果在62例胃黏膜肠上皮化生病例中,内镜诊断38例,诊断准确率为61.29%。相比较中度与轻度,重度胃黏膜肠上皮化生组内镜诊断准确率更高( P<0．05)。相比较Ⅰ型与Ⅱ型,Ⅲ型胃黏膜肠上皮化生组内镜诊断准确率更高(P<0．05)。结论内镜诊断胃黏膜肠上皮化生的最主要特征为胃黏膜肠上皮化生的形态特点,且内镜诊断准确率与胃黏膜肠上皮化生程度及分型呈正相关。

A Correlation Study between Pathological Diagnosis and Endoscopic Diagnosis for Intestinal Metaplasia in Gas-tric Mucosa

Objective To analyze correlation between pathological diagnosis and endoscopic diagnosis for intes-tinal metaplasia in gastric mucosa. Methods Clinical data of 62 patients with intestinal metaplasia in gastric mucosa during October 2012 and October 2014 was retrospectively analyzed. Characteristics of intestinal metaplasia in gastric mu-cosa were observed under endoscopy, and then diagnostic results were compared between the two methods. Results In the 62 patients with intestinal metaplasia of gastric mucosa, 38 patients were diagnosed with endoscopic diagnosis, and diagnostic accuracy rate was 61. 29%. Compared with those of mild and moderate patients, diagnostic accuracy rate of severe patients with intestinal metaplasia in gastric mucosa was higher (P<0. 05);compared with those of type I andⅡpatients, diagnostic accuracy rate of typeⅢpatients with intestinal metaplasia in gastric mucosa was higher (P<0. 05). Conclusion The most important characteristic of intestinal metaplasia in gastric mucosa with endoscopic diagnosis is morphological, and diagnostic accuracy rate with endoscopic diagnosis is positively correlated with degree and typing of intestinal metaplasia in gastric mucosa.
