社区健康管理对老年糖尿病综合控制效果的分析

目的评价对社区老年糖尿病患者实施健康管理的效果。方法对合肥市某社区卫生服务中心建档的94例老年糖尿病患者进行为期1年的健康管理。每3个月随访一次,对生活方式进行指导,比较管理前后空腹血糖、血压、血脂的水平及达标率变化。结果实施健康管理1年后,老年糖尿病患者的空腹血糖、收缩压、舒张压与体质指数的水平均较前明显降低(P0.05),高密度脂蛋白胆固醇明显升高(P0.05)。空腹血糖达标率由51.1%上升至61.7%,血压达标率由34.0%上升至44.7%,空腹血糖、血压2项同时达标率由12.8%上升至53.2%。结论社区健康管理对于老年糖尿病的综合控制有明显效果。

Efficacy of community health management in comprehensive control of diabetes mellitus in the elderly

Objective To evaluate the efficacy of health management for senile diabetes mellitus patients in community.Methods Free annual physical examination performed on 94 cases of senile diabetes mellitus patients in community for consecutive two years,while carrying out health management.Fasting blood glucose,lipids,blood pressure and rates of reaching the goal were monitored in follow-up visits.Results In 94 patients,the 2nd year,fasting blood glucose,systolic blood pressure,diastolic blood pressure,BMI levels were significantly lower compared with the 1st year（P 0.05）.HDL cholesterol was significantly increased（P 0.05）.Level of fasting glucose and blood pressure became more likely to reach the goal,51.1% to 61.7% and 34.0% to 44.7% respectively.Moreover,rate of reaching both goals of fasting glucose and blood pressure rose to 53.2% from 12.8%.Conclusion Community health management has a demonstrable effect in controlling diabetes mellitus in the elderly.
