特发性膜性肾病不同病理分期及血压水平尿蛋白谱的差异

目的探讨不同病理分期、不同血压水平特发性膜性肾病(idopathic membranous nephropathy,IMN)尿蛋白谱的差异。方法分析在我院进行初始治疗的IMN患者47例的临床资料,比较不同病理分期、不同血压水平患者尿蛋白谱的差异。结果血压正常组IgG、α1-微球蛋白(α1-MG)、β2微球蛋白(β2-MG)值均低于血压升高组(P<0.05或P<0.01);两组白蛋白(Alb)、转铁蛋白(TRF)值差异无统计学意义(P>0.05)。病理Ⅰ～Ⅱ期患者的TRF、IgG、α1-MG、β2-MG值均低于Ⅲ～Ⅳ期患者(P<0.05或P<0.01);两组Alb值差异无统计学意义(P>0.05)。结论不同血压水平和不同病理分期的IMN患者尿蛋白谱存在差异,病理改变越严重,尿蛋白含量越高。

Differences of urine protein profile in idiopathic membranous nephropathy in different pathological staging and BP

Objective To study differences of urine protein profile in idiopathic membranous nephropathy （IMN）in different pathological staging and BP. Methods The clinical data of forty-seven patients with IMN were analyzed. Urine protein profile of pa- tients in different pathological staging and different blood pressure levels was compared. Results The values of IgG,α1-MG,β2-MG of the patients with normal blood pressure were lower than those of the patients with higher blood pressure （ P 〈 0.05 or P 〈 0. 01 ）. The values of Alb, TRF of the patients in both groups had no statistical significance（ P 〉 0.05 ）. The values of TRF,IgG,α1-MG, β2-MG in Ill-IV were higher than I -II （ P 〈0.05 orP 〈0.01）. The value of Alb of the patients in both groups had no statistical signifi- cance（ P 〉 0. 05 ）. Conclusion Urine protein profile in IMN in different pathological staging and BP had differences. The more se- vere the pathology is, the higher the urine protein level is.
