2型糖尿病患者餐后血糖与左室结构及功能的关系探讨

摘    要:目的探讨2型糖尿病(T2DM)患者餐后血糖与左室结构及功能的关系。方法应用彩色多普勒超声诊断仪测定62例餐后血糖控制欠佳(A组)和58例餐后血糖控制良好(B组)的2型糖尿病患者的左室结构和功能,比较2组间的差异;对62例餐后血糖控制欠佳组的2型糖尿病患者进行降糖干预,追踪观察1年,观察干预前后左室结构及功能的动态变化。结果入组时餐后血糖控制欠佳组(A组)患者出现左室结构改变,左室心肌重量增加,舒张功能减退,与餐后血糖控制良好组(B组)患者比较,差异有统计学意义(P〈0.05)。A组患者经过1年的强化降糖治疗,左室心肌重量减少,左室重构明显纠正,舒张功能改善,与治疗前比较,差异有统计学意义(P〈0.05)。结论餐后高血糖是T2DM心脏并发症的危险因素。

Abstract:Objective To research the relationship between postprandial blood glucose level and left ventricular structure and function in type 2 diabetes mellitus(T2DM).Method The left ventricular structure and function in 62 cases with poorly controlled postprandial blood glucose and 58 cases with well controlled postprandial blood glucose were measured and compared.62 T2DM patients with poorly controlled post prandial blood glucose were intervened by glucose controlling.The changes of the left ventricular structure and function before and after the intervention were observed and followed up for 1 year.Results Compared with the well controlled postprandial blood glucose group,the left ventricular structure changed and the weight of left ventricular muscle grew in the mal-controlled postprandial blood glucose group(P<0.05).After 1 year of strict glucose control therapy,the left ventricular muscle weighed less and the systolic function improved in poorly controlled postprandial blood glucose group compared with before intervention(P<0.05).Conclusion High postprandial blood glucose is one of the risk factors of cardiac complication inT2DM.
