一期手术治疗前列腺增生并发膀胱结石、腹股沟疝23例疗效分析

目的探讨同期进行经尿道前列腺剜除、经尿道电切镜下膀胱碎石及腹股沟疝无张力修补术治疗前列腺增生合并膀胱结石和腹股沟疝的可行性和临床效果。方法对本院从2009年2月～2012年6月收治的23例前列腺增生合并膀胱结石和腹股沟疝的患者同期进行经尿道前列腺剜除、经尿道电切镜下膀胱碎石及腹股沟疝无张力修补术资料的回顾性分析,总结23例患者的术前资料、手术资料、术后情况及随访结果。结果 23例患者均顺利完成手术,围手术期无严重并发症,手术时间:120～210 min,平均:(150±35)min,术中出血:150～650 ml,平均:(280±62)ml,术后有1例患者出现轻微尿失禁,2周后恢复,术后最大尿流率:16～35 ml/s,平均(23±3.5)ml/s,术后无切口感染病例,术后复查B超或腹平片未见残石。结论同期进行经尿道前列腺剜除、经尿道电切镜下膀胱碎石及腹股沟疝无张力修补术的手术方案为前列腺增生同时合并膀胱结石、腹股沟疝的患者提供了最佳的治疗方案,避免了患者多次手术痛苦,创伤小,并发症少且减轻经济负担。

SINGLE- STAGE SURGICAL TREATMENT OF BPH WITH BLADDER STONES INGUINAL HERNIA IN 23 CASES

Objective To discuss the feasibility and clinical effect of simultaneous transuretheral enucleation of prostate, transurethral cystolithotripsy and tension - free inguinal hernia repair for the treatment of BPH complicat- ed with bladder stone and inguinal hernia. Methods Through retrospectively analyzing the data of 23cases with BPH complicated with bladder stone and inguinal hernia underwent simultaneous transurethral enucleation of prostate, tran- surethral cystolithotripsy and tension - free inguinal hernia repair, we conclude the data of the pre - operation, intrao- peration, postoperation and the result of the follow- up. Results 23cases were all performed successfully, no seri- ous complication. The operating time 120 - 210 rain, average （ 150± 35） rain; bleed volume 150 - 650 ml, average（280±62）ml; urinary incontinence happened on one case postoperactively, and recovered after 2 weeks; postopera- tive peak - flow 16 - 35 ml/s, average （23 ±3.5 ） ml/s; no incision infection case; no bladder residual stone. Con- clusion Simultaneous transurethral enucleation of prostate, transurethral cystolithotripsy and tension - free inguinal hernia repair is the best treatment for the patient with BPH complicated with bladder stone and inguinal hernia, it has the advantage of avoiding second - time surgery, less damage, less complication and less money.
