经鼻CPAP在新生儿呼吸衰竭中的应用观察与护理

目的探讨经鼻持续气道正压通气(CPAP)在治疗新生儿呼吸衰竭的疗效。方法将我院2011年12月～2012年12月期间102例轻、中度呼吸衰竭新生儿随机分为对照组和观察组,其中对照组51例采用头罩吸氧治疗及相应护理,观察组51例采用经鼻CPAP无创呼吸机给氧治疗及相应护理,观察分析两组患儿2 h后的血气情况及临床疗效并加以对照分析。结果对比观察组患儿治疗前后的血气情况,治疗后氧分压(PaO2)、氧饱和度(SPO2)和pH值明显改善且差异具有显著性(p<0.05);对比两组临床治疗有效率,观察组明显高于对照组,两组比较差异具有显著性(p<0.05)。结论经鼻CPAP治疗新生儿呼吸衰竭是可行的,可以明显提高患儿的临床疗效,值得在临床推广应用。

THE APPLICATION AND NURSING OF INTRANASAL CPAP IN NEONATE WITH FAILURE OF RESPIRATION

Objective To investigate the clinical application and nursing of intranasal CPAP （continuous positive airway pressure） in the treatment of neonate with failure of respiration. Methods 102 neonate cases with mild and moderate respiratory failure were divided into control group （ n = 51 ） and observation group （ n = 51 ）, the cases in control group were treated by inhalation oxygen with facemask, the cases in observation group were used in- tranasal CPAP respectively and appropriate nursing was made at the same time. Then compare the results of blood gas analysis after 2 hours and clinical efficacy. Results After the treatment, the PaO2 ,SPO2 and PH value in observation group were superior than that before the treatment, there was significant difference （p 〈 0. 05 ） ;the effective rate of treatment in observation group was higher than that in control group obviously, the difference of two groups comparison has statistic significance（p 〈 0. 05）. Conclusion The intranasal CPAP in the treatment of neonate with failure of res- piration was feasible and with higher clinical effective rate, it is worthy of application widely.
