完整结肠系膜切除术在左半结肠癌根治术中的临床应用价值

目的 探究完整结肠系膜切除术(CME)在左半结肠癌根治术中的临床应用价值.方法 选取接受左半结肠癌根治术的76例患者为研究对象,分为实验组和对照组,每组38例.实验组给予完整结肠系膜切除术(CME),对照组给予传统根治术治疗,比较2组患者的临床手术效果及术后机体情况.结果 实验组平均活检淋巴结数目(18.2±5.6)个,明显多于对照组的(12.9±2.7)个(P<0.05).实验组患者术中出血量为(114.9±11.4)mL,明显少于对照组的(134.9±11.8)mL(P<0.05).对照组和实验组术后总并发症发生率分别为18.4%和13.2%,差异无统计学意义(P>0.05);2组术后1年生存率比较,差异无统计学意义(P>0.05),实验组患者术后3年生存率(94.7%)明显高于对照组的71.1%(P<0.05).结论 采用完整结肠系膜切除术治疗左半结肠癌临床效果显著,可减少患者术中出血量,提高术后活检淋巴结的数目,保证切除系膜的完整性,利于患者预后,值得临床推广应用.

Clinical Application Value of Complete Mesocolic Excision in Radical Resection of Left Colon Cancer

Objective To explore the clinical application value of the complete mesocolic excision ( CME) in the radical resection of left colon cancer .Methods 76 patients with radical resection of left colon cancer were divided into the experimental group and the control group ,each group with 38 cases.The experimental group was treated with CME ,the control group was trea-ted with conventional surgery ,the clinical effect and postoperative body condition of the 2 groups were compared .Results The average number of lymph node biopsy in the experimental group was (18.2 ±5.6),which was significantly more than the control group (12.9 ±2.7)(P<0.05);the amount of bleeding in the experimental group (114.9 ±11.4) mL was significantly less than that of the control group (134.9 ±11.8) mL (P<0.05);postoperative complication rates of the control group and the ex-perimental group were 18.4%and 13.2%,but the difference between the 2 groups was not statistically significant (P>0.05);1-year survival rates of the 2 groups had no significant difference (P>0.05),3-year survival rate of the experimental group was 94.7%,which was significantly higher than the control group 71.1%(P<0.05).Conclusion CME in the treatment of left co-lon cancer has remarkable efficacy ,it can reduce the amount of bleeding during surgery ,improve the number of postoperative biop-sy lymph nodes ,and ensure the integrity of the mesentery ,it is conducive to the prognosis of patients ,and it is worthy of clinical application .
