彩色多普勒超声联合弹性超声检查在宫颈占位性病变中的应用价值

摘    要:目的:探讨彩色多普勒超声的Adler分级法、弹性超声的弹性评分法及两者联合应用在宫颈占位性病变中的诊断价值.方法:将三种超声检查方法对140例宫颈占位性病变的诊断结果与病理结果相对照,绘制ROC曲线,确定截点值,并分别计算三种超声检查手段的敏感度(Se)、特异度(Sp)、准确度(ACC).结果:140例宫颈占位性病变中良性组63例,恶性组77例.Adler分级法的敏感度为85.71%,特异度为71.43%,准确度79.29%,曲线下面积为0.84;弹性评分法的敏感度为75.32%,特异度为82.54%,准确度78.57%,曲线下面积为0.85.联合分级法的敏感度为84.44%,特异度为90.47%,准确度87.14%,曲线下面积为0.91.结论:弹性超声和彩色多普勒超声在诊断宫颈恶性占位性病变中的诊断效能无明显差异,但两者联合应用后的联合分级法的诊断效能优于两者单独应用时的诊断效能.

Abstract:Objective:To compare the diagnostic accuracy of Adler grade,elasticity score and combination grade for cervical lesions.Methods:140 patients (benign 63 cases,malignant 77 cases) who underwent Color Doppler ultrasonography and elastosonography were involved as cervical lesions,using histology as the golden standard.We can calculate the sensitivity (Se),the specificity (Sp),and accuracy rate (ACC).Results:Adler grade showed the Se,Sp,ACC were 85.71%,71.43% and 79.29%,respectively.The area under the ROC curve was 0.84.Elasticity score showed the Se,Sp,ACC were 75.32%,82.54% and 78.57 %,respectively.The area under the ROC curve was 0.85.Combination grade showed the Se,Sp,ACC were 84.44%,90.47% and 87.14%,respectively.The area under the ROC curve was 0.91.Conslusion:The diagnostic efficiency of Color Doppler ultrasonography and elastosonography were equal,however,using both can increase the the diagnostic efficiency in cervical lesions.
