病灶残腔灭活治疗软骨肉瘤的近远期效果分析

目的 探讨病灶残腔灭活治疗软骨肉瘤的近远期效果.方法 收治软骨肉瘤患者72例,根据治疗方法的不同分为观察组与对照组各36例,对照组采用手术治疗,观察组采用病灶残腔灭活治疗,观察与记录两组的近远期预后.结果 对照组平均手术时间为(290.42±56.24)min,出血量为(1622.94±451.92) ml;而观察组平均手术时间为(182.44±44.92) min,出血量为(432.11±111.84)ml,观察组均明显低于对照组(P＜0.05).观察组术后3个月的深部感染、深静脉血栓、血肿、皮缘坏死等并发症发生率为8.3％,对照组为36.1％,观察组明显低于对照组(P＜0.05).观察组与对照组术后3个月的肢体功能优良率分别为36.1％和8.3％,观察组的优良率明显高于对照组(P＜0.05).随访至今,观察组与对照组的中位生存时间分别为(78.24±11.48)个月和(59.24±10.42)个月,观察组的中位生存时间明显长于对照组(P＜0.05).结论 病灶残腔灭活治疗软骨肉瘤具有很好的微创性,在近期能提高肢体功能,降低并发症的预防,在远期能延长患者的生存时间,有很好的应用价值.

Short-term and Long-term Efficacy of Lesion Cavity Inactivation in the Treatment of Chondrosarcoma

Objective To investigate the short-term and long-term efficacy of lesion cavity inactivation in the treatment of chondrosarcoma.Methods 72 chondrosarcoma patients were selected and were divided into the observation group and the control group with 36 patients in each group,according to the different treatment methods.The control group was treated by operation,the observation group was given the lesion cavity inactivation,short-term and long-term efficacy of the 2 groups were observed and recorded.Results The average operation time of the control group was (290.42 ± 56.24) min,the amount of bleeding was (1622.94-± 451.92) ml;and the average operative time of the observation group was (182.44 ± 44.92) min,the amount of bleeding was (432.11 ± 111.84)ml,the observation group were significantly lower than the control group (P ＜ 0.05).The incidence rates of deep infection,deep vein thrombosis,hematoma and skin edge necrosis 3 months after surgery in the observation group was 8.3 ％,which was significantly lower than the control group,36.1％ (P ＜ 0.05).The good rate of limb function in the observation group and the control group 3 months after surgery were 8.3％ and 36.1,respectively,the observation group was significantly higher than that of the control group (P ＜ 0.05).Up to now,the median survival time of the observation group and the control group were (78.24 ± 11.48) months and (59.24 ± 10.42) months.The median survival time of the observation group was significantly higher than that of the control group (P ＜ 0.05).Conclusion Lesion cavity inactivation for chondrosarcoma is minimally invasive,it can improve the limb function and reduce the complications in the short-term,and it can prolong the survival time of patients in the long-term,it has good application value.
