18F-FDG PET/CT 显像对颅内占位性病变的诊断价值

摘    要:目的:评价全身18F-脱氧葡萄糖(18F-fluorodecxyglucose,18F-FDG)正电子发射断层扫描(positron emission tomography,PET/CT)显像在颅内占位性病变中的诊断价值.方法:回顾性分析143例颅内占位性病变患者的全身18F-FDG PET/CT 显像及临床随访结果.结果:本组143例患者中,PET/CT诊断脑胶质瘤10例、脑膜瘤7例、淋巴瘤2例、病变性质不明或其它类型肿瘤16例.脑转移瘤108例,确诊原发灶96例,未明确原发灶12例,原发灶检出率为88.9%,其中原发灶为肺癌的65例,占67.7%,消化系统来源恶性肿瘤17例,占17.7%,生殖系统来源恶性肿瘤3例,占3.1%,不同来源脑转移瘤间差异无统计学意义.结论:颅内占位性病变病人18F-PET/CT显像可有效发现脑内病灶和脑转移瘤中的原发灶,并可大致区分原发灶类型,脑转移瘤来源于肺恶性病变居多.

Abstract:Objective:To evaluate diagnostic value of whole-body 18F-fluorodecxyglucose 18F-FDG positron emission tomography imaging in brain accounted for lesions.Methods:Retrospectively analyse 143 patients with intracranial space occupying lesions of the brain and body PET/CT 18F-FDG imaging and clinical follow-up results.Results:In 143 cases of patients,10 cases of PET/CT in the diagnosis of glioma,meningioma in 7 cases,lymphoma in 2 cases,unknown,or other types in 16 cases.In 108 cases of brain metastases,96 cases of primary tumors were confirmed,12 cases were not clear,but the detection rate was 88.9%,including 65 cases deriving from lung cancer (67.7%),17 cases of the digestive system(17.7%),3 cases of reproductive system(3.1%).Conclusion:Intracranial space occupying lesions in patients 18F-PET/CT imaging effectively find primary brain tumors and metastases,and can roughly distinguish the pathological type and evaluate the therapeutic effect.Brain metastasis tumor derived from malignant lung lesions more.
