慢性中性粒细胞白血病伴多发性骨髓瘤1例合并文献复习

目的 分析慢性中性粒细胞白血病(CNL)伴多发性骨髓瘤的临床病理特征.方法 回顾性分析1例CNL伴多发性骨髓瘤患者的临床病理资料,并结合相关文献,进行分析.结果 患者中性粒细胞显著上升,脾脏肿大,中性粒细胞碱性磷酸酶积分增高,无Ph染色体及BCR/ABL融合基因,骨髓粒系有明确增生,未见病态造血和纤维化改变,排除类白血病反应等其他可能导致中性粒细胞增多的因素,合并多发性骨髓瘤.结论 CNL临床少见,其发病有一定特殊性,具体机制及干预方法需要进一步研究.

Chronic Neutrophilic Leukaemia Associated with Multiplemyeloma:Case Report and Litererature Review

Objective To study the clinicopathological features of chronic neutrophilic leukaemia ( CNL) complicated with multiplemyeloma .Methods One case diagnosed as CNL complicated with multiple myeloma was reported and the related literature were reviewed to provide basis for diagnosis and treatment of such diseases .Results The patients were characterized by significant increase of neutrophils ,splenauxe,high neutrophil alkaline phosphatase score ,absence of Ph chromosome and BCR-ABL fusion gene,obcious granulocytic hyperplasia of bone marrow ,without dyshaematopoiesis or fibrosis changes .excluding the factors such as leukemoid reactions which may cause neutrophilic leukocytosis ,complicated with multiple myeloma .Conclusion CNL is rare in clinic.Its attack is special.The specific mechanism and intervention methods need further study.
