经右胸入路食管癌根治术治疗食管中下段癌的疗效及安全性分析

目的 探讨经右胸入路食管癌根治术治疗食管中下段癌的疗效及安全性.方法 220例食管中下段癌患者,均实施食管癌根治术治疗,按入路方式不同分为2组.对照组经左胸入路,观察组经右胸入路.结果 观察组手术时间、失血量、术后留置胸管时间、住院时间、并发症发生率高于对照组,差异有统计学意义(P<0.05);观察组清扫淋巴结数目(19.24±6.63)枚、肿瘤成功切除率(89.09%),均高于对照组[(11.24±4.63)枚、70.91%],差异有统计学意义(P<0.05);观察组生存时间较对照组延长,局部复发、远处转移率降低,差异有统计学意义(P<0.05).结论 经右胸入路食管癌根治术治疗食管中下段癌疗效确切,具有良好的淋巴结清扫效果,可延长患者无瘤生存时间,安全可靠,值得推广.

Efficacy and Safety of Esophageal Cancer Radical Resection Via Right Thoracic Approach in the Treatment of Middle and Lower Esophageal Cancer

Objective To analyze the efficacy and safety of esophageal cancer radical operation in the treatment of mid-dle and lower esophageal cancer by right thoracic approach. Methods 220 patients with middle and lower esophageal cancer were divided into 2 groups according to the different treatment methods. The control group via left thoracic approach,the observa-tion group via right thoracic approach. Results The operation time,blood loss,postoperative indwelling chest tube time,hospital stay,incidence of complications in the observation group were significantly higher than those of the control group,the difference was significant (P<0. 05);In the observation group,the number of lymph node dissection (19. 24 ± 6. 63),tumor resection rate (89. 09%) were higher than the control group[(11. 24 ± 4. 63),70. 91%],the difference was significant (P<0. 05);Com-pared with the control group,the survival time of the observation group was longer than that of the control group,and the difference was significant (P<0. 05). Conclusion The right thoracic approach in esophageal cancer radical surgery for esophageal cancer is effective,has a good effect in lymph node dissection,it can prolong progression-free survival,and it is safe and reliable. It is worth promoting.
