摘    要:目的:研究18F-FDG PET/CT对肾上腺高代谢灶的诊断价值.方法:收集我院2013年9月到2015年10月行PET/CT检查的38例患者共47个肾上腺高代谢灶,所有病灶确诊依靠手术病理或临床随访证实,临床随访时间为6个月~2年;将所有病灶按确诊结果分为恶性组与非恶性组,并行半定量及ROC曲线法分析.结果:恶性组共23个病灶,非恶性组共24个病灶,两组之间SUVmax有显著差异(P<0.001).受试者工作曲线分析,恶性组与非恶性组ROC曲线下面积为0.981,获得的最适界点为4.45,诊断效能较高,以此为临界值,18F-FDG PET/CT显像鉴别肾上腺高代谢灶良恶性的灵敏度100%,特异度83.3%,准确度91.5%.结论:18F-FDG PET/CT显像能较准确的诊断肾上腺高代谢灶的性质,具有较高的诊断效能.

Abstract:Objective:To investigate the diagnostic value of 18F-FDG PET/CT of adrenal hyper-metabolic foci.Methods:All 38 patients from Sep 2013 to Oct 2015(47 lesions)with adrenal hyper-metabolic foci ere underwent 18F-FDG PET/CT,all lesions were confirmed by the surgical pathology or clinical follow-up(6 months to 2 years).According to the confirmed results,all lesions were divided into malignant group(23)and non-malignant group(24).The semi quantitative and receiver operating characteristic(ROC) analysis were employed to define the threshold value for the ability of SUVmax.Results:There was a highly significant difference in SUVmax between malignant group and non-malignant group(P<0.001).The area under the curve of ROC for malignant group and non-malignant group was 0.981,the threshold value of SUVmax was 4.45 for differentiating the malignant lesion and non-malignant lesion of adrenal,and its sensitivity,specificity and accuracy were 100%,83.3%,and 91.5%.Conclusion:18F-FDG PET/CT imaging can give more accurate diagnosis for the lesion with hyper-metabolic foci,and has a high diagnostic efficacy.
