静吸复合麻醉和全凭静脉麻醉对行腹腔镜胃癌根治术患者血气及免疫功能的影响

目的 研究静吸复合麻醉和全凭静脉麻醉对行腹腔镜胃癌根治术患者血气及免疫功能的影响.方法 选择80例胃癌患者,均采取腹腔镜胃癌根治术治疗,按照麻醉方法的不同分为观察组和对照组,观察组采用静吸复合麻醉,对照组采用全凭静脉麻醉.观察2组麻醉前后的免疫功能,以及CO2气腹前后血气分析的变化.结果 术毕2组的NK、CD3+、CD4+和CD4+/CD8+水平均较麻醉前明显降低(P<0.05),且对照组的下降幅度更为明显(P<0.05);术后72 h 2组的免疫功能指标均较术毕时明显升高(P<0.05).观察组气腹前后的血气分析无明显差异(P>0.05),对照组气腹后pH值明显降低(P<0.05),CO2分压、碱剩余和CO2总量明显升高(P<0.05),且观察组气腹后的血气分析与对照组之间有明显差异(P<0.05).结论 静吸复合麻醉较全凭静脉麻醉,对行腹腔镜胃癌根治术患者的免疫功能影响较小,患者术中血气分析指标平稳.

Effect of Balance Anesthesia and Total Intravenous Anesthesia on Blood Gas and Immune Function of Gastric Cancer Patients Treated with Laparoscopic Radical Gastrectomy

Objective To investigate the effect of balance anesthesia and total intravenous anesthesia on blood gas and immune function of gastric cancer patients treated with laparoscopic radical gastrectomy.Methods 80 cases of patients with gas-tric cancer were divided into 2 groups according to the different methods of anesthesia ,the observation group used balance anes-thesia,the control group used total intravenous anesthesia.The immune function of the groups before and after anesthesia ,and the change of blood gas analysis before and after CO 2 pneumoperitoneum were observed.Results The NK,CD3+,CD4+and CD4+/CD8+of the 2 groups were significantly decreased (P<0.05)and the control group was more obvious (P<0.05);the im-mune function indexes of the 2 groups 72 h after surgery were significantly higher (P<0.05);the blood gas analysis of the obser-vation group before and after pneumoperitoneum had no significant difference (P>0.05),the pH of the control group increased significantly(P<0.05),CO2 partial pressure,base excess and the amount of CO 2 decreased significantly(P<0.05).Conclusion The effect of balance anesthesia on patients undergoing laparoscopic radical gastrectomy is better than total intravenous anesthe -sia,the immune function was suppressed and the blood gas analysis index was stable.
