食管癌术后肺部感染的多因素分析

目的:分析食管癌术后发生肺部感染的危险因素,为临床制定干预措施提供参考依据.方法:回顾性分析121例食管癌切除病例的临床资料,分为感染组(39/121)和无感染组(82/121).采用单因素、多因素分析围手术期与肺部感染的相关因素.结果:术后发生肺部感染39例(32.2％),液气胸21例(17.4％),呼吸衰竭5例(4.1％),死亡1例(0.8％).单因素分析:FEV1/FVC、高血压史、切口、单/双腔气管插管、人工气胸、手术当日引流量与之相关;多因素分析:双腔气管插管单肺通气、术后当天引流量多为独立危险因素.结论:双腔插管单肺通气为术后肺部感染的危险因素,术后引流量多、术前肺功能受损为协同因素.

Multivariate analysis of postoperative pulmonary infection about esophageal carcinoma

Objective:To analyze risk factors of postoperative pulmonary infection of esophagectomy,and provid reference of clinical interventions.Methods:With retrospectively analysing clinical data,121 cases of esophagectomy patients,were divided into postoperative pulmonary infection group (39/121) and non-postoperative pulmonary infection group (82/121).Univariate and multivariate analysis were performed in perioperative factors correlation with postoperative pulmonary infection.Results:39 cases of postoperative pulmonary infection have been found in 121 cases (32.2％),with pleural effusion or pneumothorax 21 cases (17.4％),respiratory failure 5 cases (4.1％),1 patient died (0.8％).Univariate analysis showed FEV1/FVC,history of hypertension,surgical incision,single or double-lumen endotracheal intubation,artificial pneumothorax,the day after surgery drainage had statistically significant difference.Multivariate analysis showed dual-lumen endotracheal intubation,the day after surgery drainage are considered to be independent factors.Conclusion:Double lumen cannula and one lung ventilation maybe the main risk factor of postoperative pulmonary infection,and postoperative drainage,preoperative impaired lung function are important co-factors.
