超声造影与CT对胰腺癌的诊断价值

目的 比较CT联合彩超和单独超声检查对胰腺癌的诊断准确率.方法 87例胰腺癌患者均经过腹部CT和(或)超声(US)检查.以病理结果和12个月的随访结果作为参考标准,比较CT+ US和单独使用CT诊断的阳性率、敏感性、特异性和诊断准确率(AUC);比较CT+ US与单独使用CT的活检阳性值(PPV).结果 87例胰腺癌患者当中,CT+ US诊断40例,单独US诊断15例,单独CT诊断13例,19例未被诊断.单独使用CT的诊断率为14.94％,单独使用US的诊断率为17.24％,而CT+ US的诊断率提高到45.97％.联合诊断敏感性高这77.5％,特异性高达89.4％.结论 胰腺CT联合彩超可明显提高胰腺癌的诊断准确率.

Diagnostic Value of CEUS and CT for Pancreatic Cancer

Objective To compare the diagnostic accuracy of CT combined with color doppler ultrasound and single ultrasound examination in pancreatic cancer.Methods 87 patients with pancreatic cancer underwent abdominal CT and/or ultrasound (US) examination.Sensitivity,specificity,and diagnostic accuracy (AUC) were compared between CT + US and CT alone and pathologic findings and 12-month follow-up were used to compare the CT + US with CT biopsy alone Positive value (PPV).Results Of the 87 patients with pancreatic cancer,40 were diagnosed by CT + US,15 by US alone,13 by CT alone,and 19 not diagnosed.The diagnostic rate of CT alone was 14.94％,and the diagnostic rate of CT + US was 45.97％,while that of US was 17.24％.The sensitivity of the combined diagnosis was 77.5％ and the specificity was 89.4％.Conclusion Pancreatic CT combined with color doppler ultrasound can significantly improve the diagnostic accuracy of pancreatic cancer.
