超声对乳腺癌患者术前分期的诊断价值

目的 探讨超声对乳腺癌患者术前分期诊断的应用价值.方法 于术前1周,对80例乳腺癌患者进行超声分期、临床触诊分期,术后1周进行病理分期.以病理分期诊断结果作为"金标准",对比超声、触诊的诊断结果.结果彩色多普勒测得的病灶大小与病理测量值相比,P>0.05;临床触诊测得的病灶大小与病理测量值相比,P<0.05.T分期中,超声的诊断符合率为93.75%,高于临床触诊的诊断符合率(78.75%).彩超诊断腋窝淋巴结转移敏感性75.00%、特异性93.18%、阴性预测值82.00%、阳性预测值90.00%;临床触诊腋窝淋巴结转移敏感性66.67%、特异性76.47%、阴性预测值52.00%、阳性预测值80.00%.结论 超声对乳腺癌进行术前分期、肿瘤大小,诊断符合率、灵敏度、特异度、阴性预测值、阳性预测值均高于临床触诊,值得推广应用.

Diagnostic Value of Ultrasound on Preoperative Staging of Breast Cancer Patients

Objective To explore the application value of ultrasound on preoperative staging diagnosis of breast cancer patients.Methods 1 week before surgery ,ultrasonic staging and clinical palpation staging were performed for 80 patients with breast cancer .A week after surgery ,the pathological staging was performed and its results were used as golden standard to compare the diagnostic results between ultrasound and palpation .Results Lesion size and pathologic measurement were compared ( P>0.05) and measured by color doppler ultrasound .While they were compared (P<0.05) and tested by clinical palpation.In t-staging,the diagnose accordance rate of ultrasound (93.75%) was higher than that of clinical palpation (78.75%).Metastatic lymph node sensitivity (75.00%),specificity (93.18%),negative predictive value (82.00%) and positive predictive value (90.00%) were measured by color doppler ultrasound .Axillary lymph node sensitivity (66.67%),specificity (76.47%),nega-tive predictive value (52.00%) and positive predictive value (80.00%) were tested by clinical palpation .Conclusion Ultra-sound on preoperative staging ,tumor size,diagnose accordance rate ,sensitivity,specificity,negative predictive value and positive predictive value of breast cancer are higher than clinical palpation ,it is worthy of promoting application .
