摘    要:目的探讨原发性高血压患者凌晨血压增高(MBPS)与早期肾功能损害的关系。方法原发性高血压患者252例,根据MBPS差值分为MBPS组(≥23.6mmHg,64例)与非MBPS组(<23.6mmHg,188例),分别检查血糖、血脂、血与尿β2微球蛋白(β2-MG)及尿微量白蛋白(mAlb)、动态血压监测、心电图、心脏二维超声、尿常规等,比较2组之间动态血压参数及血与尿β2-MG、尿mAlb的差异。结果MBPS组凌晨收缩压、凌晨舒张压、平均动脉压、脉压、心率、血与尿β2-MG及尿mAlb明显高于非MBPS组,2组差异有统计学意义(P<0.01)。结论原发性高血压患者中MBPS者较非MBPS者更容易出现早期肾功能损害。

Abstract:Objective To investigate the relationship between morning blood pressure surge (MBPS) and the early stage of renal function damage in patients with essential hypertension. Methods According to the levels of MBPS, tntally 252 consecutive essential hypertensive patients were divided MBPS group (MBPS ≥ 23.6 mm Hg, 64 cases)and non MBPS group( MBPS 〈 23.6 nun Hg, 188 cases). The indexes, including ambulatory blood pressure monitoring, the serum and urine β2 MG, urine mAlb, the levels of blood BUN and Cr were measured and compared. Result In MBPS group, the indexes were significant higher than non-MBPS group (average P 〈 0.01 ), including morning systolic blood pressure (SBP), mean arterial pressure (MBP), pulse pressure (PP), morning heart rate, serum and urine β2-MG and urine mAlb. Conclusion It suggested that the essential hypertension with MBPS tended to damage renal function than those without MBPS.
