非外伤性颅脑手术中急性脑膨出的诊治

目的　探讨非外伤性颅脑手术中急性脑膨出的发生原因和防治措施。方法　回顾性分析 10例颅脑手术中出现急性脑膨出病人的临床资料。结果　 10例术后恢复良好 8例 ,中残 1例 ,死亡 1例。结论　非外伤性颅脑手术中急性脑膨出的原因有颅内血肿形成、脑牵拉损伤及脑缺血缺氧 ;维持平稳麻醉、避免过度降低颅压和脑牵拉 ,及时探查处理颅内血肿改善脑血供是防治急性脑膨出的有效措施。

Acute brain swelling during non-traumatic craniotomy

Objective To investigate the causes and treatment of acute brain swelling during non-traumatic craniocerebral operation.Method The clinical data of 10 patients who had acute brain swelling during their craniocerebral operation are reviewed,the causes and the treatment of the acute brain swelling analyzed.Result Eight patients have excellent recovery,1 patient moderate defecated and 1 died postoperatively.Conclusions The causes of acute brain swelling during non-traumatic craniocerebral operation include intracranial haematoma,brain retracted injury and cerebral anoxia/ischemia.Prevention and treatment of acute brainswelling include an uneventful anesthesia,avoidance of brain retroactive injury and overdrainage of cerebrospinal flow,and evacuation of intracranial haematoma immediately.
