高敏C-反应蛋白和胰岛素抵抗在冠心病患者血清中的变化

目的探讨高敏C-反应蛋白（hs-CRP）和胰岛素抵抗（IR）及与冠状动脉粥样化性心脏病（冠心病,CHD）患者冠状动脉病变的相关性。方法选择经冠状动脉造影证实的CHD患者共75例,其中稳定型心绞痛（SAP）26例,不稳定型心绞痛（UAP）30例,急性心肌梗死（AMI）19例,另选冠状动脉造影正常的30例作为对照组。结果CHD患者血浆hs-CRP水平和IR高于对照组（P〈0.05）;CHD患者中,AMI组和UAP组hs-CRP和胰岛素抵抗指数（IRI）高于SAP组（P〈0.05）;CHD患者Gensini积分与IRI、hs-CRP,IRI与hs-CRP均呈正相关（P〈0.01）。结论hs-CRP和IR可能与斑块的不稳定状态有关,两者相互影响、相互作用。

The changes of high sensitivity C-reactive protein and insulin resistance in the serum of patients with coronary heart disease

Objective To investigate the relationship between high sensitivity C-reactive protein as well as insulin resistance and coronary heart disease.Methods 75 patients with coronary heart disease confirmed by coronary angiography were enrolled in the study,in whom there were 26 cases of stable angina pectoris,30 cases of unstable angina pectoris and 19 cases of acute myocardial infarction. At the same tiime 30 subjects with normal coronary angiography were enrolled as a control group. Results The levels of high sensitivity C-reactive protein in plasma and insulin resistance index in patients with coronary heart disease were significantly higher than those of control group （P0. 05）. The levels of high sensitivity C-reactive protein and insulin resistance index in patients with acute myocardial infarction and unstable angina pectoris were significantly higher than those of stable angina pectoris group. Gensini score was positively correlated with high sensitivity C-reactive protein and insulin resistance index in patients with coronary heart disease,and high sensitivity C-reactive protein was positively correlated with insulin resistance index（P0.01）. Conclusion High sensitivity C-reactive protein and insulin resistance may be related to the instability of the plaque.
