| 摘 要： | 目的探讨翻修柄假体在高龄股骨转子间粉碎性骨折中的临床应用。方法2005年5月-2008年5月选用翻修柄人工股骨头置换治疗股骨转子间粉碎性骨折15例。骨折按Evens分型，Ⅲ型12例，Ⅳ型3例。术中注意保留大小粗隆部位的骨折块，争取将其解剖复位，用钢丝固定，安放翻修柄人工股骨头，翻修柄在股骨髓腔内有足够的长度，以便保持假体的稳定性。结果15例术后均获6-36个月随访，平均18．2个月。住院时间平均14天，术后13例35天完全负重行走，1例因其他内科疾患扶拐行走；术后并发髋关节疼痛1例；术后1年死亡1例，余14例健在。结论翻修柄用于高龄粉碎性股骨转子间骨折的治疗，能增加股骨柄的稳定性，术后恢复快，可早期下地活动，并发症少。
