外科急腹症B超诊断分析—附98例报道

目的 评估超声在外科急腹症诊断中的价值。方法 用超声对98例外科急腹症病人进行检查。观察肝脾双肾大小，形态，包膜完整性，实质内情况；胆囊大小，囊壁厚度，囊内及颈部情况；胆总管粗细及其内有无结石；左右肝内胆管有无结石或扩张。观察右下腹或右侧腹腔内有无指状急性阑尾炎声像图。结果 10例完全性肝脾破裂病人表现肝脾包膜不完整，实质内探及不均质中等回声光团，外形不规则，边界不清，43例胆囊炎，胆石症病人表现胆囊肿大，壁厚毛糙，本组多数胆囊颈部发现结石嵌顿。十例胆总管中下段梗阻病人，上段扩张，伴左右肝同胆管扩张，6例在胆总管中段或下段内找到结石。24例急性阑尾炎病人在右下腹腔内找到指状低回声光团。6例病人仅有中上腹痛或脐周痛，病程最短4小时。结论 超声是诊断外科急腹症的一种有效方法。

Analysis of ultrasonic diagnosis in the acute abdomen-98 cases report

Purpose To evaluate the value of ultrasonography in the diagnosis of acute abdomen. Methods Ninety - eight cases acute abdomen had been examined in the liver, spleen, kidney,gallbladder and other region by ultrasonography.The size,shape,capsule and substantials were observed. Results Capsules of ten cases of hepatic and splenic were ruptured and its substantials were uneven. Foxty - three cases of cholecystitis and cholelithiasis were found because gallbladder swelled and the wall coarsened. Most of them were calculus choking in the neck of the gallbladder. Ten cases whose common bile duct were widen. Six cases were found that calculus were exist in the lower part of common bile duct. Twenty - four cases of acute appendicitis were found due to the finger form of sonogram were found in the right lower abdomen. Six cases of them only had pains in the central abdomen or the umbilical region. Conclusion Ultrasonography is a valuable diagnostic method for acute abdomen.
