含腓浅血管的带蒂皮瓣修复胫前创面

目的:介绍含腓浅血管的带蒂皮瓣修复胫前创面的手术方法及临床效果。方法:根据14侧成人下肢标本及一条断腿的解剖学观察,设计了含腓浅血管的L形带蒂皮瓣及舌状带蒂皮瓣。1993年4月至1996年10月,应用该术式修复胫前创面11例,创面最大5×4cm,最小2×2cm。结果:术后皮瓣全部成活,切口Ⅰ期愈合9例,Ⅱ期愈合2例。随访6个月到2年,皮瓣在受区生长良好,创面未复发。结论:本皮瓣含有腓浅血管及较宽的皮蒂两套供血系统,较岛状皮瓣及任意带蒂皮瓣血供明显增加,不但有利于皮瓣成活,又有利于感染控制,适于创面直径在5.0cm以下,创面周围疤痕组织较多或感染未完全控制的创面的修复。

Pedicle flap with superficial peroneal vessel for wound in front of tibia

Objective: To introduce the operative method and clinical result of the pedicle flap with superficial peroneal vessel for wound in front of tibia. Methods ? According to the anatomic study on 14 legs of adult cadavers and one amputated lower limb, the L-shape and tongue-shape pedicle flap with superficial peroneal vessel were designed-From Aril 1993 to October 1996,11 cases who had a wound in front of tibia which was 2x2cm to 5x4cm,were treated with the operative ways. Results:All the flaps survived, 9 healed with I stage and 2 with I stage. Follow-up was from 6 months to 2 years,The flaps were growing well and the wound had no recurrence. Conclusions: the flap contains two kinds of blood supply system from suerficial peroneal vessel and a wider skin pedicle,So its blood supply is very reliable and suit-abe to repair the wound in front of tibia which is smaller than 5-0 cm in diameter .scarring and inflaming round the wound
