腹腔镜辅助阴式手术治疗子宫内膜癌的效果分析

目的分析腹腔镜辅助阴式手术治疗子宫内膜癌的临床效果。方法60例子宫内膜癌患者,按照随机数字表法分为观察组和对照组,各30例。对照组患者采用单纯腹腔镜手术治疗,观察组患者采取腹腔镜辅助阴式手术治疗。对比两组患者术中出血量、手术时间、肛门排气时间、留置导尿管时间、住院时间及术后并发症发生情况、术后生存率。结果观察组术中出血量(121.23±8.12)ml少于对照组的(187.45±9.55)ml,手术时间(160.31±8.52)min、肛门排气时间(2.12±0.51)d、留置导尿管时间(2.56±0.23)d及住院时间(8.11±2.12)d均短于对照组的(168.25±8.32)min、(2.54±0.46)d、(3.14±0.33)d、(12.88±2.36)d,差异有统计学意义(P<0.05)。观察组术后并发症发生率明显低于对照组,差异有统计学意义(χ²=5.455,P<0.05)。观察组术后1、2、3年生存率分别为96.67%、86.67%、73.33%,均明显高于对照组的80.00%、63.33%、40.00%,差异有统计学意义(χ²=4.043、4.356、6.787,P<0.05)。结论采用腹腔镜辅助阴式手术治疗子宫内膜癌患者,其手术操作更加简单、安全,可有效减少术后并发症,对提高患者生存率具有重要作用,值得临床推广应用。

Effect analysis of laparoscopic assisted vaginal surgery in the treatment of endometrial cancer

Objective To analyze the clinical effect of laparoscopic assisted vaginal surgery in the treatment of endometrial cancer.Methods A total of 60 patients with endometrial cancer were divided into observation group and control group according to random numerical table,with 30 cases in each group.The control group was treated with laparoscopic surgery,and the observation group was treated with laparoscopic assisted vaginal surgery.The intraoperative blood loss,operation time,anal exhaust time,indwelling catheter time,hospitalization time,incidence of postoperative complications and survival rate were compared between the two groups.Results The intraoperative blood loss(121.23±8.12)ml of the observation group was less than(187.45±9.55)ml of the control group,and operation time(160.31±8.52)min,anal exhaust time(2.12±0.51)d,indwelling catheter time(2.56±0.23)d and hospitalization time(8.11±2.12)d were shorter than(168.25±8.32)min,(2.54±0.46)d,(3.14±0.33)d and(12.88±2.36)d of the control group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications of the observation group was obviously lower than that of the control group,and the difference was statistically significant(χ²=5.455,P<0.05).The survival rate of 1,2 and 3 years after operation of the observation group were 96.67%,86.67%and 73.33%,which were obviously higher than 80.00%,63.33%and 40.00%of the control group,and the difference was statistically significant(χ²=4.043,4.356,6.787;P<0.05).Conclusion Laparoscopic assisted vaginal surgery in the treatment of endometrial cancer patients can reduce the incidence of postoperative complications,and the operation is more simple,safe,and plays an important role in improving the survival rate of patients,which is*worthy of clinical promotion and application.
