MRI对膝关节韧带损伤的诊断价值分析

目的探讨磁共振成像(MRI)对膝关节韧带损伤的诊断价值。方法选取120例疑似膝关节韧带损伤患者,分别采用CT、MRI以及关节镜诊断,以关节镜诊断结果为金标准,对比不同诊断方式韧带撕裂诊断符合情况及敏感度和特异度。结果关节镜诊断结果显示:韧带正常患者31例,韧带部分撕裂患者40例,韧带完全撕裂患者49例。CT诊断韧带正常符合率、韧带部分撕裂符合率和韧带完全撕裂符合率分别为74.19%(23/31)、82.50%(33/40)、77.55%(38/49),MRI诊断韧带正常符合率、韧带部分撕裂符合率和韧带完全撕裂符合率分别为93.55%(29/31)、97.50%(39/40)、97.96%(48/49),MRI诊断韧带正常符合率、韧带部分撕裂符合率和韧带完全撕裂符合率均高于CT诊断,差异具有统计学意义(P<0.05)。MRI诊断韧带撕裂的敏感度和特异度分别为97.75%(87/89)和93.55%(29/31);CT诊断韧带撕裂的敏感度和特异度分别为79.78%(71/89)和74.19%(23/31),MRI诊断韧带撕裂的敏感度和特异度均显著高于CT诊断,差异具有统计学意义(P<0.05)。结论MRI和CT均能有效诊断出患者韧带损伤后伴随撕裂程度,而MRI在诊断时还能表现出更高的敏感度和特异度,较CT诊断结果更精准,临床应用价值更高。

Value analysis of MRI in the diagnosis of knee ligament injury

Objective To discuss the value of magnetic resonance imaging(MRI) in the diagnosis of knee ligament injury. Methods A total of 120 patients with suspected knee ligament injury were selected and diagnosed by CT, MRI, and arthroscopy. The results of arthroscopy were used as the gold standard to compare the coincidence, sensitivity and specificity of different diagnostic methods for ligament tear diagnosis. Results Arthroscopic diagnosis showed that there were 31 cases with normal ligament, 40 cases with partial ligament tear and 49 cases with complete ligament tear. The coincidence rate of CT diagnosis of normal ligament, partial ligament tear, and complete ligament tear were 74.19%(23/31), 82.50%(33/40), and 77.55%(38/49), respectively, which were 93.55%(29/31), 97.50%(39/40), 97.96%(48/49), respectively of MRI. The coincidence rate of CT diagnosis of normal ligament, partial ligament tear, and complete ligament tear of MRI were all higher than those of CT, and the difference was statistically significant(P<0.05). The sensitivity and specificity of MRI in diagnosing ligament tear were 97.75%(87/89) and 93.55%(29/31), respectively;the sensitivity and specificity of CT in diagnosing ligament tear were 79.78%(71/89) and 74.19%(23/31). The sensitivity and specificity of MRI in the diagnosis of ligament tears were significantly higher than that of CT diagnosis, and the difference was statistically significant(P<0.05). Conclusion Both MRI and CT can effectively diagnose the degree of ligament tear after injury, and MRI can also show higher specificity and sensitivity in the diagnosis, which is more accurate than CT diagnosis, and has higher clinical application value.
