64层螺旋CT在肺部结节病影像学诊断中的价值

目的探讨在诊断肺部结节病患者时采用64层螺旋CT检查的临床价值,观察其影像学特征。方法30例肺部结节病患者,均经活检检查确诊为肺部结节病,对其进行64层螺旋CT检查,将检查结果与活检检查结果进行对比,分析其诊断符合率,并观察肺部结节病的影像学特征。结果以活检检查结果作为诊断金标准,64层螺旋CT检查结果符合率为100%(30/30),未出现漏诊以及误诊的情况。30例患者均显示淋巴结肿大,淋巴结短径中间值为(16.52±3.62)mm;隆突下间隙淋巴结明显肿大,淋巴结短径中间值为(16.87±2.63)mm。采用64层螺旋CT检查,患者均有明显的淋巴结肿大强化特征,且增强后CT值增加32~43 HU。患者中3例存在少量胸腔积液,2例存在少量心包积液,胸膜改变发生率为16.7%(5/30)。23例患者发生肺内改变,其中,16例患者存在肺内散在和胸膜下多发结节,4例发生肺磨玻璃样变,2例发生肺纤维化,1例发生肺内单发结节,肺内改变发生率为76.7%(23/30)。结论64层螺旋CT诊断肺部结节病符合率较高,操作方便,可为临床诊断及对症治疗提供可靠的参考依据。

Value of 64-slice spiral CT in imaging diagnosis of pulmonary sarcoidosis

Objective To discuss the clinical value of 64-slice spiral CT in the diagnosis of patients with pulmonary sarcoidosis and observe its imaging characteristics.Methods 30 patients with pulmonary sarcoidosis were diagnosed as pulmonary sarcoidosis by biopsy.They were examined by 64-slice spiral CT,and the results of examination were compared with those of biopsy.The diagnostic coincidence rate was analyzed,and the imaging characteristics of pulmonary sarcoidosis were observed.Results Taking biopsy results as the gold standard for diagnosis,the coincidence rate of 64-slice spiral CT was 100%(30/30),and there was no missed diagnosis or misdiagnosis.30 patients all showed lymph node enlargement,the median value of the short diameter of the lymph node was(16.52±3.62)mm;the subcarinal space lymph node was significantly enlarged,and the median value of the short diameter of the lymph node was(16.87±2.63)mm.64-slice spiral CT examination showed that the patients all had obvious features of lymph node enlargement and enhancement,and the CT value increased by 32-43 HU after enhancement.Among the patients,3 cases had a small amount of pleural effusion,and 2 cases had a small amount of pericardial effusion.The incidence of pleural changes was 16.7%(5/30).23 patients had pulmonary changes,including 16 patients with scattered and subpleural nodules in the lungs,4 cases with groundglass changes in the lungs,2 cases with pulmonary fibrosis,and 1 case with a single nodule in the lungs.The incidence of changes in the lungs was 76.7%(23/30).Conclusion 64-slice spiral CT has high coincidence rate,in the diagnosis of pulmonary sarcoidosis and the operation is convenient,which can provide reliable reference for clinical diagnosis and symptomatic treatment.
