扩张型心肌病患者血清sICAM-1与TNF-α水平及临床相关性研究

目的检测扩张型心肌病患者血清可溶性细胞间黏附分子．1（soluble intercellul aradhesion molecule1，sICAM-1）和肿瘤坏死因子-α（tumornecrosisfactor-α，TNF-α）水平，探讨其与扩张型心肌病的发生发展的关系及其临床意义。方法应用酶联免疫双抗体夹心法（ELISA）测定了25例扩张型心肌病患者和15例正常对照组的血清sICAM-1和TNF-α水平。结果扩张型心肌病组血清sICAM-1和TNF-α水平明显高于对照组（P〈0．01）；心功能Ⅱ级组血清sI-CAM-1和TNF-α水平高于对照组（P〈0，05）；心功能Ⅲ级组血清sICAM-1和TNF-α水平明显均高于心功能Ⅱ级组（P〈0，01），心功能Ⅳ级组血清sICAM．1和TNF．d水平明显高于心功能Ⅲ级组（P〈0，01）。血清sICAM-1和TNF-α水平存在显著的正相关（r：0，807，P〈0，001）。结论扩张型心肌病患者血清中sICAM-1、TNF-α高表达，并且随着心衰加重，外周血清中siCAM-1和TNF-α的水平逐渐升高。

Level of Serum Soluble ICAM-1 and TNF-α and its Correlation to Clinical Features in Patients with Dilated Cardiomyopathy

Objective To detect the level of soluble intercellular adhesion molecule （slCAM） and tumor necrosis factor-α （TNF-α） in serum, and evaluate its correlation to the changes of cardiac function in patients with dilated cardiomyopathy （ DCM ）. Methods The levels of slCAM-1 and TNF-α in the serum from 25 patients with DCM and 15 normal individuals were measured by enzyme-linked immunosorbent assay （ELISA）. Results Significant increase in the concentrations of serum slCAM- 1 and TNF-α were found in the patients with DCM as compared with the controls （P 〈 0.01 ）. The mean level of slCAM-1 and TNF-α related significantly to the cardiac functional class ： the level of patients with class Ⅱ was higher than that of normal control （ P 〈 0.05 ） ,class Ⅲ than class Ⅱ , and class IV than class Ⅲ. The level of slCAM-1 and TNF-α were positive correlated in the patients with DCM （r = 0.807,P 〈 0. 001 ）. Conclusions Serum levels of slCAM-1 and TNF-α were high in patients with DCM, and increase with the development of heart failure.
