| 摘 要： | 目的 分析胰十二指肠切除术后胰瘘的原因及预防经验。方法 回顾分析1994～2003年行胰十二指肠切除术的63例患者的临床资料，对影响胰瘘发生的因素和采取的治疗方法进行总结。结果 胰瘘与吻合技术有关，胰瘘的发生与残胰断面的处理以及胰实质的坚硬度、胰管的直径有关；胰管内支撑引流可预防PD术后胰瘘。结论 胰管内支撑引流和良好的胰切面血供是预防PD术后胰瘘的关键。

| Abstract: | Objective To study the prevention and reasons of pancreatic fistula after pancreaticoduodenectomy.Methods A retrospective analysis was made on 63 patients with pancreaticoduodenectomy from 1994 to 2003,in order to evaluate the rcasons of pancrcatic fistula.Results Occurrence of pancreatic fistula was related with pancreaticojejunal anastomosis,manages of pancreatic section,stiffiness of pancreatic parenchyma and pancreatic duct diameter.Pancreatic duct endoprosthetic drainage to prevent pancreatic fistula after pancreaticoduodenectomy.Conclusion Panereatic duct endoprosthetic drainage and nicer bloodstream purvey of pancreatic section are a key to prevent pancreatic fistula after pancreaticoduodenectomy. |
