后路治疗胸腰段脊柱骨折椎弓根螺钉位置的探讨

摘    要:目的探讨后路治疗胸腰段脊柱骨折术中的椎弓根螺钉植入的位置。方法回顾性分析应用RF、AF内固定系统治疗胸腰段脊柱骨折38例。术前测量X线片上椎弓根与上一椎体同侧下关节突的位置关系,确定椎弓根钉入钉点,在C臂X线机透视下,应用后路切开复位后,RF、AF椎弓根系统内固定。结果术后行X线片检查,所有患者手术均获成功,术后无神经症状加重或出现新的神经症状者,无内固定失败及伤椎高度明显丢失者。结论术前测量胸腰段脊柱骨折X线片上椎弓根与同侧上一椎体下关节突相互位置,为选择椎弓根螺钉的入钉点提供了一种新方法。

Abstract:Objective To study the angle of pedicle screw insertion for thoracolumbar vertobrae fractures treated with posterior procedurcs. Methods 38 cases of thoracolumbar vertobrae fractures treated with RF, AF spinal internal fixator system were studied retrospectively. To measure the sito realationship which the pedicle realatc to the inferion articular facet of adjacent superior vertebral body on the x-ray before operation and determine the site and angle ofpedicle screw. After reduction and fixation with RF, AF spinal internal fixator systems were performed under fluoroscopic guidance. Results All patients obtained successful surgery without neurological complications or notable corrcction loss or implant failure on the x-ray after operation. Conclusion To measure the site relationship which the pedicle realate to the inferior articular facet of adjacentsuperior vertebral body on the x-ray for thoracolumbar vertebrae fractures before operation is a promissng procedure for the site and angle ofpedicle screw insertion.
