脑室出血72例临床分析

目的探讨脑室出血的治疗方法。方法 72例脑室出血患者。随机分为治疗组和对照组,每组36例。治疗组应用改良脑室外引流术＋腰大池引流术;对照组应用传统脑室外引流术＋腰椎穿刺术,观察两组疗效。结果治疗组无颅内感染病例;继发交通性脑积水1例,经侧脑室腹腔分流术治疗后痊愈。对照组颅内感染6例,经抗感染治疗后5例痊愈,1例死亡;继发交通性脑积水6例,经侧脑室腹腔分流术治疗后好转,两组并发症比较差异有统计学意义（P〈0.05）;两组疗效比较差异有统计学意义（P〈0.05）。结论应用改良脑室外引流术＋腰大池引流术治疗脑室出血,能够减少颅内感染,降低死亡率、改善预后。

Clinical analysis of 72 cases of ventricular hemorrhage

Objective To explore the treatment method of ventricular hemorrhage. Methods A total of 72 cases with ventricular hemorrhage were randomly divided into treatment group and control group, and each group contained 36 cases. The treatment group received improved external ventricular drainage and lumbar cistern drainage, while the control group was treated hy conventional external ventricular drainage and lumbar puncture. The curative effects were observed. Results There was no ventricular infection occurred in the treatment group, and there was 1 cases of secondary communicating hydrocephalus, which healed after ventricle-peritoneal shunt operation. In the control group, there were 6 cases of ventricular infection, and 5 of them were cured by antiinfective therapy while the other 1 case died. There were 6 cases of secondary communicating hydrocephalus, which improved after treated by ventricle-peritoneal shunt operation. There were significant differences between the two groups of the complications （P〈0.05） and curative effects （P〈0.05）. Conclusion The application of improved external ventricular drainage and lumbar cistern drainage in the treatment of ventricular hemorrhage can reduce the incidence of intracranial infection and mortality, and improve the prognosis.
