急性肾功能衰竭中肾小管上皮细胞脱落及DNA损伤的意义

摘    要:目的 探讨肾小管上皮细胞脱落及DNA损伤在急性肾功能衰竭(Acute renal faiure,ARF)中的意义。方法 分别以5、10ml/kg50%甘油注射于Wistar大鼠后肢肌肉,造成不同程度的ARF,1、3、6、12、24、48、96h测定血BUN、Cr;膀胱穿刺取尿液台盼蓝 拒染法计数脱落细胞,单细胞凝胶电泳测定DNA损伤。结果 尿中脱落肾小管上皮活细胞计数及DNA损伤变化明显早于BUN、Cr,与肾功能相关;二者变化趋势相似,有一定相关性。结论 细胞脱落、DNA损伤是ARF肾小管上皮细胞损伤“多米诺骨牌”中的重要环节,其变化与损伤程度相关,有助于早期发现ARF的发生并判断其程度。

Abstract:Objective To investigate the role of tubular cell exfoliation and DNA damage in the development of aute renal failure (ARF), especially at the early stage.Methods ARF was induced by glycerol injection intramuscularly at different dosages in Wistar rats. Their blood and urine specimens were obtained 1? 3? 6? 12? 24? 48?96 h after injection. Then Cr and BUN in serum were detected. Urinary exfoliated tubular cell counting and DNA damage were studied by trypan blue dye exclusion and single cell gel electrophoresis assay (SCGE) respectively. Results After glycerol injection, the significant changes of exfoliated tubular cell count and DNA damage presented earlier than Cr and BUN in serum. Their tendencies were similar and showed positive relationship with the degree of renal function damage. Conclusions The results suggest that cell exfoliation and DNA damage are important mechanisms of tubular cell injury in ARF. They can be used not only to find ARF at the early stage but also to evaluate the degree of ARF.
