原发性十二指肠球部腺癌的诊治--附4例误诊病例分析

目的探讨原发性十二指肠球部腺癌的临床特点及诊治方法.方法对北海市人民医院普外科1994-2002年误诊的4例十二指肠腺癌的临床资料进行回顾性分析.结果本组4例术前、术中均误诊,从而导致误治.结论对40岁以上有消化性溃疡症状的患者要警惕该病的存在,内镜活检或术中活检是确诊的依据.手术切除是首选的治疗方法,应根据肿瘤的分期选择术式.化疗和放疗未能显著延长生存期.

Diagnosis and Treatment of Primary Adenocarcinoma at Duodenal Bulb:A Misdiagnosis Report of 4 Cases

Objective To explore the clinical manifestations, effective methods of diagnosis and treatment of primary adenocarcinoma at duodenal bulb.Methods Four cases misdiagnosed primary adenocarcinoma at duodenal treated by authors were reported and analysed.Results four cases were misdiagnosed preoperatively and mistreated intraoperatively.Conclusions Doctors must pay attention to the patients who are older than 40 years with the symptom of peptic ulcer.Endoscopic examination or intraoperative biopsy are necessary.The best method of treatment is surgical excision according to the tumors grade.Chemotherapy and radiotherapy do not significantly improve survival rate.
