后路手术治疗胸腰段椎体骨折49例

目的:总结应用后路钉棒系统内固定治疗胸腰段椎体骨折的临床效果。方法:回顾性分析后路椎弓根钉棒系统治疗胸腰段椎体骨折49例,观察脊柱功能改善,椎体前后缘高度变化,cobb角的矫正程度及并发症发生情况。结果:随访6-34个月,椎体平均高度由术前的前缘37.4%和后缘的81.2%恢复到术后的前缘95.7%和后缘97.2%,cobb角由术前平均28.3°恢复为术后平均4.7°。脊髓神经功能明显改善。按Frankel分级除A级3例无改善外,其余均有1-3级的神经功能恢复。结论:后路钉棒系统具有手术相对简单,操作方便,固定可靠等优点,是胸腰段椎体骨折的有效治疗方法之一。

Treatment for 49 patients with thoracolumbar fractures operated through posterior approach

Objective:To summarize the therapeutic effect of pedicle screw fixation system through posterior approach on patients with thoracolumbar fractures.Methods:The clinical data of 49 patients with thoracolumbar fractures treated by pedicle screw fixation system through posterior approach were retrospectively analyzed in which function improvement of vertebral column,height change of anterior and posterior border of the centrum,correction of cobb angle and the complication were respectively observed.Results:Following up 6～34 months,the average height of anterior border improved from 37.4%of pre-operation to 95.7%of post-operation,posterior border from 81.2%to 97.2%,and cobb angle also recovered from 28.3°of pre-operation to 4.7°of post-operation.Except 3 cases,the function of spinal cord recovered 1～3 rank of frankel classification.Conclusion:Pedicle screw fixation system through posterior approach has the merits of simplicity,convenience and reliable fixation,which is one of the efficient method to treat thoracolumbar fractures patients.
