摘    要:目的探讨慢性特发性血小板减少性紫癜(ITP)患者外周血活化T淋巴细胞表面CD154表达水平、凋亡率的变化及两者之间关系。方法选择我院慢性ITP患者40例和健康对照者21例,用淋巴细胞分离液分离外周血单个核细胞(PBMC),CD3单抗活化标记其中T淋巴细胞,体外培养6h后用流式细胞仪检测其表面CD154的表达水平;用Annexin V/PI试剂盒2h内测活化T淋巴细胞凋亡率。结果(1)ITP组活化T细胞表面CD154表达水平明显低于正常对照组(2.85%±2.22%vs6.17%±3.01%,P〈0.01)。(2)ITP组活化T细胞凋亡率水平明显低于对照组(27.640±11.65%VS45.71%±12.37%,P〈0.01)。(3)ITP组外周血T细胞比例与正常对照组无明显差异(51.74%±12.860VS49.81%±10.79%,P〉0.05)。(4)活化后T淋巴细胞表面CD154表达水平与其凋亡率呈正相关(r=0.462,P〈0.05)。(5)外周血活化T淋巴细胞凋亡率水平与外周血血小板计数呈正相关(r=0.680,P〈0.01)。结论慢性ITP患者活化T淋巴细胞表达的CD154水平降低,抑制了活化T淋巴细胞凋亡,导致自身免疫功能紊乱,外周血血小板减少。

Abstract:Objective To detect the expression of CD154 on peripheral blood activated T-lymphocyte and the apoptosis rate change in the patients with idiopathic thromboeytopenic purpura (ITP), investigate the relationship of them. Methods Isolated the PBMC by ficoll from peripheral blood then use CD3-mAb labeling and actived T-lymphocyte in 40 cases of ITP patients and 21 cases of healthy control, simultaneously label CD154-mAb, measured expression of CD154 after incubation 6 hours by flow eytometry, measured T-lymphocyte apoptosis rate with Annexin V/PI kit within 2 hours by flow eytometry. Results The expression of CD154 on activation T-lymphocyte (%) in ITP was lower that in healthy control (2. 85%±2. 22% vs 6. 17%±3. 01%, P〈0. 01). The activated T-lymphocyte apoptosis rate in ITP was lower than that in healthy control (27. 64%±11.65% vs 45. 71±12. 37%P〈 0. 01) . The rate of peripheral blood T cell have no difference in ITP and healthy control group (51.7%±12. 86% vs 49. 81%±10. 79%,P〉0. 05). The expression of CD154 on activated T-lymphocyte was positive correlation with it's apoptosis rate (r=0. 462,P〈0. 05). The apoptosis rate of activated T-lymphocyte was positive correlation with peripheral blood platelets count (r=0. 680, P〈 0. 01). Conclusion The depress expression of CD154 inhibition apoptosis of activated T-lymphocyte, induce autochthonous immune function confused, result in peripheral blood platelets count decrease.
