流行性脑脊髓膜炎病人脂肪代谢分析

目的　对流行性脑脊髓膜炎病人脂肪代谢情况进行初步探讨。方法　用CX 5型全自动生化分析仪检测了 72例流行性脑脊髓膜炎病人总胆固醇、甘油三脂、血糖及肝功能 ,其中 36例完成极期与恢复期双份血检测 ;死亡的暴发型流脑 3例死后立即行肝穿刺电镜检查。结果　与恢复期相比 ,极期甘油三脂、总胆固醇明显下降 (P <0 0 5 ) ,而血糖明显升高 (P <0 0 1) ,此外AST、LDH在极期亦显著增高 (P <0 0 5 ) ;电镜显示肝脏脂肪浸润 ;36例病人中 ,有 15例在极期出现心功能不全。结论　流行性脑脊髓膜炎病人脂肪代谢异常 ,脂肪分解增加 ,肝脏脂肪浸润 ,心功能受到影响。血甘油三脂、总胆固醇可作为判定病人病情轻重和观察疗效的指标之一。

Analysis of lipometabolism in patients with epidemic cerebrospinal meningitis

Objective To explore the lipometabolism in patients with epidemic cerebrospinal meningitis.Methods Using CX 5 full automatic biochemical instrument,we measured blood total cholesterol,trigiyceride,blood sugar and liver function in 72 patients with epidemic cerebrospinal meningitis,including 36 cases who were detected in convalescence as well as in stadium acmes.Liver puncture and electron microscopy were done immediately after death in 3 patients with fulminant form.Results A significantly reduced blood total cholesterol and trigiyceride(P<0.05) and elevated blood sugar concentration(P<0.01) were found in stadium acmes compared with convalescence,and AST and LDH levels were also elevated(P<0.05).Liver fatty infiltrations were shown by electron microscopy.Cardiac insufficiency occurred in 15 of 36 patients in stadium acmes.Conclusion There are abnormalities of lipometabolism,such as increasing lipodieresis,liver fatty infiltration,and cardiac insufficiency in patients with epidemic cerebrospinal meningitis.
