膀胱癌中MMP-2、TIMP-2的表达及其与浅表性肿瘤复发的关系

目的：研究基质金属蛋白酶－2（MMP－2）和金属蛋白酶组织抑制因子－2（TIMP－2）在膀胱癌中的表达以及它们与肿瘤临床病理因素和复发的关系。方法：用免疫组织化学SP法检测46例膀胱移行细胞癌标本中MMP－2、TIMP－2的表达，并将它们与肿瘤临床和病理参数相比较。结果：在46例膀胱癌中，MMP－2、TIMP－2的阳性率分别为47.8%和58.7%，TIMP－2在间质中的表达率为47.8%。MMP－2表达率随着肿瘤理分级、临床分期的升高而增加，而TIMP－2表达率则呈下降趋势，但在浅表性膀胱癌（Ta-T1）中，TIMP－2的表达与MMP－2相似，随着肿瘤分期分级的升高而增加，TIMP－2间质表达阳性组中浅表性膀胱癌的2年复发率显著高于表达阴性组。结论：MMP－2和TIMP－2的相互作用对于膀胱癌的侵袭发展发挥了重要作用。TIMP－2在间质中表达可作为判断浅表性膀胱癌复发的预后指标。

Expression of matrix metalloproteinase-2、tissue inhibitor of metalloproteinase-2 in transitional cell carcinoma of bladder and its correlation with the tumor recurrence

Purpose:To assess the expression of MMP-2 and TIMP-2 in bladder carcinoma and their relationship of clinicopathology and recurrence. Method:The expression of MMP-2 and TIMP-2 was analyzed immunohistochemically (SP method) in 46 patients with transitional cell carcinoma of bladder, and was compared with clinicopathologic and prognostic parameters.Result:Among 46 cases, the expression rate of MMP-2 and TIMP-2 were 47.8% and 58.7% respectively. The expression rate of TIMP-2 in the stroma was 47.8%. Positive expression of MMP-2 increased with pathological stage and grade while TIMP-2 expression tended to decrease. However, in the superficial tumors (pT a-pT 1), the expression of TIMP-2 also increased with tumor stage and grade, which was analogous with MMP-2. The 2-year recurrence rate of stromal TIMP-2 expression patients was higher than corresponding negative expression group in superficial tumors.Conclusion:The interaction between MMP-2 and TIMP-2 play a key role in the invasion of bladder carcinoma. The stromal expression of TIMP-2 would be an indicator of determining the recurrence of superficial bladder tumor.
