心电图与冠状动脉造影对诊断冠心病价值的探讨

目的探讨静息心电图与冠状动脉造影对诊断冠心病的临床价值及其相关性。方法对 876例冠心病和可疑冠心病进行12导联静息心电图和冠状动脉造影检查 ,并对其结果进行对照性分析。结果心电图表现与冠状动脉病变一致者 5 65例( 64 5 % ) ,其中心电图呈心肌缺血性改变而冠状动脉有狭窄病变者 3 91例 ( 4 4 6% ) ,心电图和冠状动脉均正常者 174例( 19 9% )。心电图表现与冠状动脉病变不一致者 3 11例 ( 3 5 5 % ) ,其中心电图正常而冠状动脉狭窄者 166例 ( 18 9% ) ,心电图呈心肌缺血性改变而冠状动脉正常者 14 5例 ( 16 6% )。结论临床上不能单纯依靠心电图来诊断冠心病 ,要根据临床表现、心电图运动负荷试验、动静态心肌断层显像或冠状动脉造影等检查 ,综合判断 ,以明确诊断

Role of ECG and coronary angiography in diagnosis of coronary heart disease

Objective To explore the role and the correlation of rest electrocardiogram (ECG) and coronary angiography (CAG) in diagnosis of coronary heart disease. Methods 876 patients with coronary heart disease (CHD) and suspected CHD underwent rest 12-lead electrocardiography and coronary angiography. Comparative analysis was performed among them. Results Electroncardiographic findings were in accordiance with the coronary pathology in 565 cases, accounting for 64.5%. Among those, 391 cases showed ischemic changes on ECG and coronary stenosis(44.6%); the other 174 cases showed both normal ECG and normal angiography(19.9%). ECG finding were inconsistence with coronary lesion in 311 cases, accounting for 35.5%; 166 cases showed normal ECG but coronary stenosis (18.9); the other 145 patients showed ischemic ECG with normal coronary angiograph(16.6%). Conclusion In clinical practice, CHD should be diagnosed with clinical symptoms, exercise stress test, resting and moving ECT, or CAG. The diagnosis of CHD should not relay on only rest ECG.
