对内科治疗无效的慢性胃炎的心理特征分析

目的：探讨内科治疗无效的慢性胃炎的心理特征。方法：将64例内科治疗无效的慢性胃炎设定为研究组与48例内科治疗有效的慢性胃炎设定为对照组，对两组进行艾森克人格、HAMD、HAMA分值、SeIL-90、功能失调性态度问卷（DAS），对两组患者认知功能、抑郁、焦虑等进行比较并将以上各值与慢性胃炎症状严重度进行相关分析。结果：研究组存在明显的神经质性人格，多为内向不稳定型，并伴有明显的焦虑抑郁情绪，抑郁焦虑分值与慢性胃炎症状的严重度呈正相关（r=0．59，F=54．98；r=0．86，F=339．98），研究组与对照组在SCL-90在躯体化、焦虑、抑郁、恐怖因子分上有显著性差异，研究组与对照组采用功能失调性态度问卷（DAS）对两组患者进行认知功能比较：研究组为132．97±17．13，对照组为105．90±21．36，t=6．15 p〈0．01；两方面比较经统计学处理均有显著差异。结论：慢性胃炎的患者多有神经质性人格，其对疾病的认知曲解、抑郁焦虑情绪对慢性胃炎的症状严重度和病程长短的有明显影响。

Psychological characteristics of patients with chronic gastritis who failed to medical treatment

Object ： To explore psychological characteristics of the patients with chronic gastritis who failed to medical treatment. Methods ： Evaluate psychological characteristics of the patients divided into two groups including 64 cases as a study group and 48 cases as a control group with Eysenck Personality . Questionnaire （EPQ）,Hamilton Depression Scale （HAMD）, Hamilton anxiety scale （HAMA） Symptom check list-90 （SCL-90） and Dysfunctional Attitude Scales（DAS） .An analysis performde to find the association between scores of scales and severity of chronic gastritis. Result：Patieuts in the study group were proved to have a tendency of nervousness with personality of introversion and instability and accompanied by evident anxiety and depressed emotion.Scores of depression and anxiety were positively correlated with the severity of chronic gastritis （r=0.59,F=54.98;r=0.86,F=339.98）.There were significant differences between the two groups on factor scores of depression,anxiety and phobic,and similar outcome detectde by a comparation of congnitive in DAS ,between these two groups（t=6.15,p 〈0.01）. Conclusion：Symptoms of chronic gastritis are closely related to patients personality most of which were characterized by nervosity, cognitive dysfunction and their depressed and anxious emotion of patients with chronic gastritis contributed to the severity and prolonged courses of chronic gastritis.
