阻塞性睡眠呼吸暂停综合征定位诊断的比较研究

目的:研究磁共振成像(MRI)及上气道压力测定两种检查方法判定阻塞性睡眠呼吸暂停综合征(OSAS)患者阻塞平面的诊断符合率.方法:20例OSAS患者(经多导睡眠监测已确诊),先后接受MRI检查及上气道压力测定,并判定阻塞平面,进行比较.结果:两种检查方法诊断符合率为50%.在判定软腭平面阻塞时,两种检查方法无显著性差异;但判定舌根平面阻塞时,上气道压力测定明显优于MRI检查.结论:上气道压力测定和MRI检查相结合,既能提高对阻塞平面判定的准确率,又能了解气道的解剖形态.

Comparative study of determining the site(s) of airway obstruction in obstructive sleep apnea syndrome

Objective:To compare magnetic resonance image(MRI) and upper airway pressure monitoring for locating the site(s) of obstruction in the upper airway. Method:Twenty patients were diagnosed as obstructive sleep apnea syndrome (OSAS ) polysomnographically and simultaneous upper airway pressure was monitored with a custom-made, soft silicone-covered catheter measuring 1.9mm in diameter. MRI was also made for these patients. Then determined the site(s) of airway obstruction and compared both methods above.Result: Identical result was obtained by MRI and upper airway pressure monitoring in ten (50%) of the patients. In other ten patients, the obstruction demonstrated with upper airway pressure monitoring could not be confirmed with MRI. There was no significant difference in determining the velopharyngeal region destruction between two methods, but had significant difference in determining the hypopharyngeal region destruction.Conclusion:We suggest that it is beneficial for diagnosis and treatment of OSAS combining with MRI and upper airway pressure monitoring.
