血清TPA、NSE、CYFRA21-1和GPI联合检测对肺癌诊断价值的探讨

目的 :探讨血清 TPA、NSE、CYFRA2 1- 1和 GPI联合检测对肺癌的临床价值。方法 :对 112例原发性肺癌患者和 76例肺良性疾病患者采用酶联免疫吸附法和化学比色法分别测定TPA、NSE、CYFRA 2 1- 1和 GPI。结果 :肺癌组 TPA、NSE、CYFRA2 1- 1和 GPI水平及检测阳性率均明显高于良性肺病及正常对照组 (P <0 .0 1)。鳞癌、腺癌和小细胞肺癌分别以 CYFRA2 1- 1和 TPA、TPA和 GPI、NSE和 GPI升幅较为明显。 1～ 4项标志物联合检测对肺癌检测的敏感性分别为 94 .6 4 %、6 4 .2 8%、4 1.0 7%和 15 .18%。结论 :TPA、NSE、CYFRA 2 1- 1和 GPI联合检测可明显提高肺癌的阳性检出率和检测特异性 ,对肺癌的早期诊断、疗效监测、预后判断有重要意义。

DISCUSSION ON THE COMBINING TESTING OF SERUM LEVELS OF TPA, NSE,CYFRA21-1 GPI FOR THE CLINICAL VALUE IN LUNG CARCINOMA

Objective:To study the clinical value for lung carcinoma by combining testing of the serum levels of TPA,NSE,CYFRA21 1 and GPI.Methods:TPA,NSE,CYFRA21 1 and GPI in serum sample of 112 cases of pateints with lung carcinoma and 76 cases of benign lung disease were determind by using ELISA and chemistry colorimetry.Results:The serum levels and the positive rate of TPA,NSE,CYFRA21 1 and GPI were higher in the lung carcinoam group than those in the benign lung diseases group with a high statistical significance ( P <0.01).CYFRA21 1 and TPA,TPA and GPI,NSE and GPI increased significantly in serum of adenocarcinoma,squamous lung carcinoam and small cell lung carcinoma respectively.The positive rates of detection one,two,three or four tumor markers were 94.64 %,64.28 %,41.07 % and 15.18 % respectively.Conclusion:The positive rate may be increased by combining test of TPA,NSE,CYFRA21 1 and GPI in lung carcinoma but not in benign lung diseases.Thus the combining test might be of high value for the early diagnosis,improving the therapeutic effect and prognosis of lung carcinoma.
