硬膜外镇痛对剖宫产术后产妇泌乳功能的影响

目的观察剖宫产术后硬膜外镇痛对产妇初乳时间及乳量的影响。方法选择硬膜外麻醉下行剖宫产术的产妇100例，随机分为两组：镇痛组和对照组各50例，镇痛组术毕行硬膜外镇痛（镇痛药物配方：0.125%罗哌卡因100ml＋芬太尼1mg；给药方法（机械泵）：负荷量5 ml，背景剂量2ml/h）。对照组术后4-6h双氯芬酸钠栓剂50 mg经直肠给药，6 h后重复1次，总量100mg。观察两组72h初乳时间及乳量情况。结果镇痛组镇痛评分（VAS评分）明显低于对照组，泌乳时间比对照组早，乳量比对照组多。结论剖宫产术后硬膜外镇痛能促进早泌乳，增加乳量。

Influence of epidural analgesia of the lactation function after cesarean section

Objective To observe the influence of postoperative analgesia with epidural analgesia on colostrum time and the quantity of lactation after cesarean section. Methods 100 parturients undergoing cesarean section under epidural analgesia were randomly divided into two groups： the epidural analgesia group（group A,n=50）and the control group（group B, n=50）. In group A, analgesia mixed fluid was injected by disposable infusion pump for postoperative analgesia. Group B was given rectal suppository of diclofenal sodium 50mg at 6 hours and 12 hours postoperatively. The colostrum time and the quantity of lactation were observed for 72 hoursResults The VAS of group A was significantly lower than that of group B（P〈0. 01）. The colostrum time was significantly earlier than that of group B（P〈0. 01）. The quantity of lactation in group A was significantly more than that of group 13 （P 〈0. 01）. Conclusion Postoperative analgesia with epidural analgesia after cesarean section can increase the quantity of lactation and promote the time of colostrum.
