摘    要:目的:观察肠外瘘病人入院初期人体组成成分的连续性变化。方法:采用多频生物电阻抗分析仪观察61例肠外瘘病人在入院10天内人体组成与血浆蛋白的变化。结果:病人体质指数与体细胞群在观察期间均有改善,但只有后者差异具显著性,而细胞外水/总体水的比例快速敏感地降至正常水平。血浆蛋白水平虽有提高,但只有前白蛋白差异具显著性。结论:肠外瘘病人入院后早期营养支持可快速改善水分分布的异常,并可以阻止体细胞群的进一步分解消耗。

Abstract:Objective To investigate the sequential changes of body composition occurring in the patients with enterocutaneous fistula after admission.Methods Sixty one patients with enterocutaneous fistula had measurements of plasma constitutive proteins and body composition by multiple frequency bioelectrical impedance analysis after admission and 10 days later.Results The body mass index and body cell mass gradually moved up over the study period,only a highly significant difference was found in body cell mass.Extracellular water(ECW)/total body water(TBW)significantly declined to near normal levels.There was a little increase of plasma constitutive protein synthesis with a significant difference in prealbumin.Conclusion The early nutritional intervention rapidly ameliorates the abnormal distribution of body water and prevents the further deterioration in body cell mass in patients with enterocutaneous fistula after admission.
