米非司酮、复方米非司酮配伍甲氨蝶呤保守治疗输卵管妊娠的疗效比较

摘    要:目的:比较米非司酮及复方米非司酮配伍甲氨蝶呤在输卵管异位妊娠药物保守治疗中的疗效。方法:选取异位妊娠病例120例,不限制β-hCG值、血清孕酮值,排除出血性休克者均予药物保守治疗。统一药物保守治疗方案,甲氨蝶呤20mg肌肉注射,1次/d,连续5d;随机选择联合米非司酮(米非司酮25mg口服,2次/d,共3d)、复方米非司酮(米非司酮30mg+双炔失碳酯5mg口服,1次/d,共2d)及单纯使用甲氨蝶呤。治疗d1、d4、d7、d11、d14分别用化学发光法检测β-hCG值、血清孕酮值。结果:米非司酮组的疗效较复方米非司酮组显著。结论:米非司酮配伍甲氨蝶呤在输卵管妊娠的药物治疗上有重要的作用,复方米非司酮不适宜用于输卵管妊娠的保守治疗。

Abstract:Objective:Compare the efficacy between mifepristone and compound mifepristone along with methotrexate in medical conservative therapy of tubal pregnancy. Method:120 ectopic pregnancy patients were selected and the level of serum β-hCG and progesterone unlimited. All cases herein were given conservative treatment with medication except patients with hemorrhagic shock. All patients were treated with methotrexate (MTX 20mg/d i.m. 5days), and randomly selected followed by mifepristone (50mg each time, twice-daily for three days), compound mifepristone (one pill orally, once a day, for 2days) and the simple use of methotrexate. The serum levels of β-hCG and Progesterone were detected on day1, 4, 7, 11, and 14 after the therapy. Results:Efficacy of mifepristone group is better than compound mifepristone group. Conclusion:Mifepristone combined methotrexate have an important role in the medical treatment of tubal pregnancy;anorethindrane dipropionate in compound mifepristone is not suitable for conservative treatment of tubal pregnancy.
