全髋置换术治疗成人先天性髋关节发育不良24例

目的:探讨成人髋臼发育不良患者行全髋关节置换术后的中近期疗效.方法:对24例成人先天性髋关节发育不良采用全髋转换术治疗,收集术前Harris评分、术后X线片、手术时间及出血量等临床数据并分析.结果:随访24～36个月.Harris 评分:术前28～65分,术后72～92分.术后无坐骨神经麻痹、下肢深静脉栓塞、切口感染和人工关节脱位等并发症,X线显示假体位置良好,未见无菌性松动和假体下沉等征象.结论:全髋置换术治疗成人先天性髋关节发育不良,近期效果良好;术前充分评估,选择合适假体,术中重建髋臼中心是手术成功的关键.

The effect of total hip replacement in the treatment of developmental dysplasia of the hip in 24 adults

Objective:To explore the near- and middle-term curative effects of total hip replacement in the treatment of developmental dysplasia of the hip(DDH) inadults.Methods:Twenty-four patients with DDH were treated with total hip replacement.The preoperative Harris scores, postoperative X-ray, operation time and bleeding volume of all cases were analyzed.Results:All patients were followed-up for 24 to 36 months.The preoperative and postoperative Harris scores were 28 to 65 and 72 to 92, respectively.The sciatic nerve palsy, deep vein thrombosis, wound infection and artificial dislocation were not be found in all patients.X-ray showed the prosthesis location was good, and no aseptic loosening and sinking of prosthesis appeared.Conclusions:The near-term curative effects of total hip replacement in the treatment of DDH of adults are good.The sufficient preoperative assessment, choosing suitable prosthesis and reconstruction of acetabulum center are the key of successful operation.
