多感官游戏训练联合常规作业治疗对脑卒中患者上肢功能的影响

摘    要:目的:探讨多感官刺激游戏训练联合常规作业治疗对脑卒中患者上肢运动及日常生活功能的影响。方法:将符合条件的60例脑卒中患者随机分为联合组和常规组各30例。联合组在接受20min常规作业治疗后,增加20min多感官刺激的游戏训练,共40min/d;常规组接受常规作业治疗40min/d,每组治疗均为5d/周,共4周。治疗前后使用简式Fugl-Meyer上肢(FMA-UL)评估上肢运动功能,改良Barthel指数(MBI)评估日常生活能力,比较治疗后满意度的问卷。结果:4周后,两组FMA-UL,MBI评分较治疗前均有明显改善(P<0.05);联合组FMA-UL评分显著高于常规组(P<0.05),但MBI评分组间并无明显差异(P>0.05);另外,联合组的治疗满意度明显高于常规组(P<0.05)。结论:多感官刺激的游戏训练联合常规作业治疗可提高脑卒中患者上肢运动功能,增加治疗的满意度,但对日常生活能力改善不明显。

Abstract:Objective:To observe the effects of multi-sensory game training combined with occupational therapy on upper extremity motor behavior and activities of daily living in stroke patients. Methods:Sixty eligible patients were randomly divided into combined group(n=30) and conventional group(n=30).The combined group received 20 minutes of routine occupational therapy and increased 20 minutes of multi-sensory stimulation for 40 min/days, while the conventional group accepted 40 minutes of occupational therapy, 5 d/week for 4 weeks in every group. FMA-UL, MBI were adopted to evaluate the motor functions and ability of daily living before and after treatment, respectively, and a questionnaire about satisfaction with training was also studied. Results: After 4 weeks, both groups showed significant improvement in FMA-UL, MBI scores compared to the baseline(P<0.05). FMA-UL score in the combined group was significantly higher than that in the conventional group(P<0.05). However, there was no significant difference about MBI scores between the two groups(P>0.05). In addition, the treatment satisfaction of the combined group was significantly higher than that of the conventional group(P<0.05).Conclusion:Combining multisensory game training with occupational therapy can improve the upper extremity motor function of stroke patients and increase the satisfaction of treatment, but the improvement of daily life ability is not obvious..
