自体脂肪提取物促进慢性创面愈合的临床研究

目的探讨应用自体脂肪提取物促进局部皮肤慢性创面愈合的效果和可能机制。方法选择北部战区总医院整形外科自2018年1月至2019年1月收治的6例全层皮肤缺损慢性创面患者,其中小腿3例,前臂2例,头颈部1例;创面面积2.3 cm×2.5 cm^6.8 cm×7.5 cm。将15 ml自体脂肪提取物,分3次局部1周内给药,之后采用InSight誖三维伤口测量记录系统测量并记录慢性创面愈合速率及并发症情况。结果6例患者无感染等并发症发生,给予自体脂肪提取物后第7天,小腿部创面平均愈合率为11.2%,前臂创面平均愈合率为12.1%,面颈部创面愈合率为13.1%。结论对比常规慢性创面的愈合率,自体脂肪提取物处理过的慢性创面并未显著加速创面愈合速率,但局部肉芽组织生长旺盛,有可能对慢性创面的远期愈合效果有改善。

Clinical study on the application of autologous adipose in the promotion of chronic wounds healing

Objective To explore the efficacy and possible mechanismof autologous adipose in the promotion of chronic wounds healing.Methods From January 2018 to 2019,6 patients with chronic wounds of full-thickness skin defects in our department were selected.There were 3 cases of chronic wounds of lower leg,2 cases of chronic wounds of forearm and 1 case of chronic wounds of head and neck.The area of wounds ranged from 2.3 cm×2.5 cm to 6.8 cm×7.5 cm.Autologous adipose of 5 ml was local administrated for 3 times within 1 week.The healing rate and complications of chronic wounds were measured and recorded by InSight three dimensional measurement system.Results No complications such as infection occurred.At 7 days after autologous adipose was given,the average healing rate of chronic wounds was 11.2%of lower leg,12.1%of forearm and 13.1%of head and neck.Conclusion Compared with the conventional healing rate of chronic wounds,the healing rate of chronic wounds after autologous adipose treated was not significantly accelerated.However,the local granulation tissue grewvigorously.The application of autologous adipose may promote the long-termefficacy and skin texture.
