宫腔镜手术与开腹子宫肌瘤剔除术治疗88例早期弥漫性子宫平滑肌瘤患者对比研究

目的:比较宫腔镜手术与开腹子宫肌瘤剔除术治疗88例早期弥漫性子宫平滑肌瘤患者的疗效。方法:选取某院2017年2月~2018年1月收治的早期弥漫性子宫平滑肌瘤患者88例,按照入院先后顺序分为两组,各44例。开腹组行开腹子宫肌瘤剔除术,宫腔镜组行宫腔镜手术,观察对比两组术中出血量、手术时间、住院天数、术后排气时间、肌瘤复发率、子宫异常率、症状改善率和并发症发生率。结果:宫腔镜组术中出血量少于开腹组,宫腔镜组住院天数和术后排气时间短于开腹组(P<0.05);两组手术时间比较无显著差异(P>0.05);两组肌瘤复发率、子宫异常率和症状改善率比较(P>0.05);两组并发症发生率比较(P>0.05)。结论:与开腹子宫肌瘤剔除术相比,宫腔镜手术治疗早期弥漫性子宫平滑肌瘤可减少术中出血量,缩短住院天数和术后排气时间,疗效确切,安全性高。

Comparative Study of Hysteroscopic Surgery and Open Uterine Myomectomy for 88 Patients with Early Diffuse Uterine Leiomyoma

Objective:To compare the efficacy of hysteroscopic surgery and open uterine myomectomy in 88 patients with early diffuse uterine leiomyoma. Methods:Eighty-eight patients with early diffuse uterine leiomyoma treated in a hospital from February 2017 to January 2018 were enrolled. According to the order of admission, they were divided into two groups,with 44 cases in each group. The open group was given open uterine myomectomy, and the hysteroscopic group underwent hysteroscopic surgery. The intraoperative blood loss, operation time, hospital stay, postoperative exhaust time, fibroid recurrence rate, uterine abnormal rate, symptom improvement rate and incidence of complications were observed and compared between the two groups. Results: The amount of bleeding in the hysteroscopic group was lower than that in the open group. The number of hospital stays and postoperative exhaust time in the hysteroscopic group were shorter than those in the open group(P<0.05);There was no significant difference in operation time between the two groups(P>0.05);The recurrence rate, abnormal uterine rate and symptom improvement rate between the two groups were compared(P>0.05);The incidence of complications were compared between the two groups(P>0.05). Conclusion:Compared with open uterine myomectomy, hysteroscopic surgery for early diffuse uterine leiomyoma can reduce the amount of intraoperative blood loss, shorten the length of hospital stay and postoperative exhaust time,with accurate curative effect and high safety.
